HIV glomerulonephritis (GN).

We studied 3 HIV-infected patients with renal failure and nephrotic syndrome, with proliferative GN with crescents, consistent with immune complex GN (ICD). HIV antibodies (Abs) were assessed by ELISA; circulating immune complexes (CIC's) were measured by C'1q assay, isolated by polyethylene glycol precipitation, subjected to gel electrophoresis and incubated with Abs to HIV antigens (Ags). Renal biopsies were acid eluted; eluates were analyzed similarly. Eluted Abs and Abs from the CICs were assessed by immunodiffusion with eluate, urinary and CIC Ags. DNA from biopsy and control tissue from non-HIV-infected patients with renal disease were tested by PCR to assess tissue presence of HIV genome. CICs were detected in each patient, composed of IgA complexed to p24 HIV Ag, an IgG Ab-gp120 HIV Ag complex, and an IgG Ab-p24 HIV Ag complex. Eluates showed identical HIV p24 Ag-anti p24 IgA Ab and HIV gp120 Ag-anti gp120 Ab complexes in the first two patients; only albumin, HIV p24 Ag and complement products were eluted from the third biopsy. Eluted Abs reacted with the HIV Ags from the isolated CICs and shed urinary HIV Ags. HIV gag and env genome were present in renal tissue from the patients, but not from negative controls. We conclude viral genome in renal tissue may effect the pathogenesis of ICD. HIV CICs may be associated with GN, by CIC deposition, or secondary to in-situ mechanisms.
