CASE-REPORT: A 65-year-old woman, followed since 1989 for a post-transfusional HIV-1 infection, was admitted in our department on January 1993 because of dyspnoea and fever. She received zidovudine therapy and inhaled pentamidine since 1991 for a significant decrease of CD4 cells count (< 0.2 x 10(9)/l). On admission, she presented with dyspnoea at rest, cyanosis, unproductive cough, fever (39 degrees C), tachycardia and rales on pulmonary auscultation. Chest X-ray revealed an interstitial pneumonitis, and blood gases showed severe hypoxemia (PO2: 41 mmHg). Blood cells count was: 7.2 x 109/l leucocytes. 126 x 10(9)/l platelets, hemoglobin level: 116 g/l, CD4 cells count: 0.07 x 10(9)/l. Blood cultures were sterile. Bronchoscopy with bronchoalveolar lavage (BAL) was negative on direct examination, but cultures of the BAL liquid revealed adenovirus (adv) type 3. Despite symptomatic treatment, the patient died of acute respiratory failure. Postmortem biopsy of the lung showed histopathological lesions evocative of viral disease (haemorrhagic alveolitis, septa necrosis), with no evidence for CMV. DISCUSSION: Adv infections in the immunocompromised hosts (transplant recipients as well as AIDS patients) are often different from those of the normal host in terms of persistence and severity of disease. Adv type 7, 11, 34 and 35 an the most commonly isolated. Recently, new serotypes were described (45-47). To our knowledge, fatal disease associated to adv 3 was not previously reported. These data suggest that adv should be added to the list of opportunistic agents in AIDS.
