Intestinal T cells have a unique state of activation and differentiation which might specifically affect or be affected by HIV infection. These effects of HIV infection on the intestinal immune system are not well characterized. Therefore we used flow cytometry to analyze lymphocytes isolated from endoscopical biopsies. METHODS: Lymphocytes were isolated by enzymatic digestion and gradient centrifugation from large intestinal biopsies of 14 HIV-infected patients at stage CDC IV and of 11 controls undergoing diagnostic endoscopy. Cells were triple-stained with fluorochrome-conjugated mabs against CD3 and various surface antigens and analyzed by flow cytometry. RESULTS: Compared with controls, the expression of CD4 (median 8%[range 1-21] vs. 53% [29-62]), CD45RO (91%[81-98] vs. 99%[93-99]), and CD29 (35%[9-68] vs. 68%[41-97]) of intestinal T cells was reduced in HIV-infected patients, and the expression of CD8 (72%[41-89] vs. 45%[28-58]) was increased. The expression of CD45RO and CD29 was reduced on CD8+ T cells (90%[80-99] vs. 98%[83-100] and 27%[7-76] vs. 74%[18-100], respectively), but was not altered on CD4+ T cells of HIV-infected patients compared with controls. DISCUSSION: Like in the circulation, there is a loss of CD4+ and an increase in CD8+ T cells in the intestinal immune system of HIV-infected patients. The decreased expression of CD45RO and CD29 in HIV infection is not due to a preferential loss of differentiated CD4+ cells, since it is predominantly seen in CD8+ T cells; it therefore indicates rather an impaired differentiation of intestinal T cells in HIV infection.
