Sample preparation parameters that affect the quantification of HIV-1 RNA level.

HIV-1 RNA quantification is becoming an important virological marker for monitoring HIV-1 infected patients. HIV-1 RNA levels in plasma and serum were determined for 10 HIV-1 seropositive individuals (CD4+ range 4 to 520). Serum Separation Tubes (SST(R), Becton Dickinson), Acid Citrate Dextrose (ACD-A VACUTAINER(R) Tubes, BD), Plasma Preparation Tubes (PPT(TM) with Citrate, BD) and Cell Preparation Tubes (CPT(TM) with Citrate, BD) were used to collect blood. All SST(R), PPT(TM) and CPT(TM) were centrifuged within one hour of blood collection and stored at room temperature. Serum/plasma aliquots were removed at 1, 3, 6 and 30 hours post blood draw. Four ACD-A blood samples from each patient were stored at room temperature and centrifuged at each time point. Plasma aliquots were stored at -70 degrees C before analysis. Assays for HIV-1 RNA were performed using a branched DNA (bDNA) assay (Chiron). The mean level of HIV-1 RNA in serum was 41% of that in citrated plasma one hour post blood collection for 8 patients (28,000 vs. 68,000 HIV RNA equivalents per mL by bDNA). The relative drop in HIV-1 RNA levels in ACD-A plasma samples over 27 hours was 31% (+/- 17%). The relative drop in HIV-1 RNA levels in the PPT(TM) was 16.6% (+/- 12%) while in the CPT(TM) it was 2.0% (+25%). Plasma is the preferred specimen for measurement of HIV-1 RNA levels. Processing specimens within a few hours of blood draw is imperative. Gel separation devices such as PPT(TM) or CPT(TM) can reduce the degradation of HIV-1 RNA levels when delays due to transport or processing are unavoidable.
