Pap smears changes in HIV(+) as compared to HIV(-) women.

Objective: To compare Pap smears over time in HIV(+) vs. HIV(-) women. Methods: Pap smears were done at entry in 273 women and every 6 months from 1989-1992 as part of a Natural History Study (R01-HD 23698-05). The women were divided as: OBP (OB HIV(+), 69); OBN (OB HIV(-), 80); GYP (non-pregnant HIV(+), 50); GYN (non-pregnant HIV(-), 74). The pre Bethesda classification is used. For this study, abnormal smears are defined as those reporting dysplasia, carcinoma in situ, or malignant cells. P values are based on the log rank test. Results: Abnormal smears were seen in 4% of both GYN and GYP groups and in 1 and 3% of the OBN and OBP groups respectively. Over time the incidence of abnormal smears reached the 75th percentile in 751 days and 552 days for the GYP and OBP arms respectively, whereas the lower bound for a C.I. of 95% was 995 days in the GYN arm (p less than .0001). Conclusion: Abnormal Pap smears occur significantly more commonly over time in HIV(+) compared to HIV(-) women, implying a greater risk for cervical intraepithelial neoplasia. Increased resources will be needed to adequately follow and treat these women to determine whether they are at increased risk for invasive cervical neoplasia.
