How do I fill out or update my JHQ, and where can I find my Training Profile?

First, access the EH&S Home Page, and click on the Training button. This will take you to the Training Home Page. Click on the JHQ Button. A screen will then appear asking for your LDAP name and password. If you don’t have a Lab email account, read the additional instructions on the screen. Press the "Log In" button and the Training Web Page will appear. Open up "Take Questionnaire" by clicking on the "+" sign.

If you are a new employee, visitor, guest, or contractor and need to fill out a new JHQ, select the "Take/Retake JHQ" button and follow the instructions.

If you are a current employee, visitor, guest, or contractor and have already taken a JHQ but need to update it, select the "Modify Existing" button.

Answer all of the questions. When you have completed/updated your JHQ, click on the bar at the bottom of the page to submit it to EH&S. An electronic copy will be forwarded to your supervisor and Division Safety Coordinator. When you have completed or updated your JHQ, a screen will appear to let you view your "Training Profile". This lists your required and recommended courses and is based on the responses given in the JHQ. Your training profile will also indicate the courses you have taken. You can print a copy if desired.

Another way to access your training profile by logging on to the training web site and opening up "Reports". You can view the training courses you’ve taken and still need to take by opening up "Your Profile".
