OBJECTIVES: 1) To examine body fat (BF) and fat-free mass (FFM) depletion in HIV infection as a function of gender and race, 2) To evaluate losses of BF and FFM using a gender- specific anthropometric relationship (triceps/subscapular skinfolds). METHODS: Body composition studies of 237 HIV- infected men and women and 577 controls, including Caucasians, African-Americans and Latinos were analyzed. Body weight (BW) and Body Mass Index (BMI) were calculated. BF and FFM were determined by dual-X ray absorptiometry. Tricep (TRI) and subscapular (SCP) skinfold-thicknesses were measured with calipers, and TRI/SCP calculated. RESULTS: Control men had higher BW, BMI and FFM and lower BF than control women. HIV+ men and women had lower BW and BMI than race and gender-specific controls. Depletions of FFM accounted for 40% of the BW difference in HIV+ men, but less than 8% in HIV+ women. TRI/SCP were higher in control women than in control men. TRI/SCP was about 14% lower in HIV+ men than in controls, while TRI/SCP was almost 50% lower in HIV+ women than in controls. Race had a smaller effect upon BW, BMI, FFM, BF, and TRI/SCP than did gender or disease (HIV). CONCLUSIONS: 1) The composition of lost weight differs in HIV+ men and women, 2) HIV+ women lose disproportionately more BF than HIV+ men, 3) Excess losses of BF and alterations in TRI/SCP in HIV+ women suggest the possibility of hypogonadism, homologous to the relationship of testosterone deficiency to excess FFM loss in HIV+ men.
