In the context of the increasing number of female sex workers (FSWs) and the growing social and health problem of HIV/AIDS in China, a cross-sectional study aimed at identifying the factors affecting condom use based on the Health Belief Model among FSWs was conducted in 1998-1999. A questionnaire survey of 236 former FSWs and 8 cases in-depth interview were used to collect data. The findings of this study showed that 15.2% of the respondents used condoms regularly, and 6.8% of the women never used condoms. The main reason of condom use was "STD prevention" (59.4%), only 14.1% of the women used condoms for "HIV/AIDS prevention". Most of the FSWs (76.8%) used condoms effectively in terms of HIV/AIDS preventing. The results of bivariate analysis indicated that the respondents who were born in rural areas, poor educated, no regular sexual partners, unknowledgeable about HIV/AIDS, and less talked about HIV/AIDS and condom use were unlikely to use condom. Perceived susceptibility to HIV infection, perceived severity of HIV infection, and perceived benefits/barriers of condom use were not statistically significant to predict condom use. In conclusion, the Health Belief Model was not good enough in understanding condom use among FSWs in this study. It is recommended that an effective and culturally sensitive approach should be used to understand and promote condom use among female sex workers in terms of HIV control in China.
