Hypothesis: specific macrophages are associated with neuropathogenesis in different locations: dementia in the CNS and peripheral neuropathy in the PNS. Background: There is a higher virus load in CNS macrophages (M0) than in PNS M0. In both tissues M0 appear involved in pathogenesis and produce toxic molecules such as cytokines and nitric oxide. Can these M0 be distinguished by surface markers? Materials and Methods: Six-micrometers cryo-sections of sciatic, tibial, sural nerves and temporal lobe were studied using standard immunohistochemistry. Nerve sections were from 4 HIV+ and 5 HIV- cases. Brain sections were from 11 HIV+ and 7 HIV- cases. Surface antigens detected include CD11c, CD14, CD68, and HLA-DR on macrophages. An immunomagnetic bead method was used to purify and culture macrophages from blood and brain. Results: In the AIDS PNS, M0 were increased and their distribution varied. CD11c+ cells predominated in tibial nerves, CD68 in sural nerves, HLA-DR+ activated cells in sciatic nerves, and CD14 in all three nerves. In AIDS temporal lobe there was an increase of CD11c, CD14, CD68, and HLA-DR+ cells. In culture these markers fluctuated independently with time. Discussion/Conclusion: Macrophages express select surface antigens differentially in different microenvironments and this may reflect differences in function and maturation. Toxic substances produced by M0 may differ by region in kind and amount produced. This approach is fundamental to elucidating mechanisms of neuropathogenesis.
