Background: Though HAART has reduced mortality in HIV, there are concerns about its long-term complications. There are no data on metabolic changes in HIV patients initiating generic HAART in India, where glucose intolerance and cardiovascular disease are common in the population. Methods: Prospective study at YRG CARE, Chennai, of ART naive HIV patients initiating NNRTI based HAART (AZT/d4t+3TC+EFV) begun in April 2003. Lipids, glucose, body shape and composition were measured at 0 & 6 months Results: 25 subjects (mean age 35 years, 64% male, mean CD4 168cells/mL, median viral load 262,000; BMI 19.2kg/m2) had anthropometric & metabolic measurements at 0 & 6 months. After 6 months of HAART there was a mean increase in CD4 of 158cells/mL & complete viral suppression (<400copies/mL). There was a significant increase (p<0.001) in total cholesterol (139+/-33 to 176+/-38mg/dL), HDL (26+/-8 to 50+/-9mg/dL) & LDL (94+/-35 to 111+/-31mg/dL) from baseline to six months, triglycerides increased from 92+/-27 to 109+/-73mg/dL (not significant). Fasting glucose at baseline(83.8+/-8mg/dL) & 6 months(85.9+/-8mg/dL) were normal. The baseline fasting insulin was high at 24+/-40muIU/mL. At 6 months 7 (28%) lost weight or were weight stable (range:0 to -1.3kg; BMI:19.2+/-3.6 kg/m2; baseline BMI:19.9+/-4 kg/m2). In this group, there was a significant (p<0.001) decrease in triceps(TSF) & subscapular(SSF) skinfolds, waist, hip & thigh circumferences of -2.7mm, -1.1 mm, -0.14cm, -1.5cm, -0.2cm respectively. Body fat and body cell mass (BCM) by BIA decreased 0.1kg & 0.5kg. In the weight gain (range 0.2-8kg; BMI: 21+/-4.2kg/m2; baseline BMI:18.9+/-4.1kg/m2) group (n=18), there was a significant (p<0.001) increase in TSF(3.4mm) & SSF(3.9mm), waist(5.7cm), hip(4.2cm) & thigh(4.5cm) circumferences and body fat(3.6kg) & BCM(1.3kg). Conclusion In an ART naive population in India initiated on HAART, changes in glucose and lipid metabolism are evident at 6 months, whether these changes will alter the risk of cardiovascular disease has to be determined.
