BACKGROUND: Functional derangement of the thyroid has been reported in association with HIV infection. Despite the rapid growth of AIDS in India, not much is known about the spectrum of thyroid lesions in patients with AIDS in our epidemiologic setup. To assess the spectrum of thyroid lesions in the Acquired Immune Deficiency Syndrome, thyroids obtained at 93 postmortems were studied. METHODS: A retrospective and prospective study from 1988 to 2002, of thyroids obtained at autopsy in 93 patients with AIDS at a tertiary India, was carried out. RESULTS: Significant pathologic lesions were identified in 33/93 (35%) cases, while thyroids in 60 (65%) cases did not show any microscopic abnormalities. Tuberculosis was the predominant finding, identified in six (6%) cases. The other pathologies identified were cryptococcosis, five (5%); cytomegalovirus, two (2%); Hashimoto's thyroiditis, three (3%); fibrosis, six (6%); goiter, three (3%); and calcification, eight (9%). Pneumocystis carinii thyroiditis and malignant neoplasms of the thyroid were not seen. CONCLUSION: Several HIV-related lesions were identified in the thyroid. Tuberculosis was the predominant infection. These lesions are rare and can occasionally result in altered thyroid function.
