Atypical intestinal mycobacteriosis mimicking Whipple's disease in a HIV-infected patient: a case report.

OBJECTIVE: To investigate the etiology of the diarrhea in a HIV-infected patient and to report the first case of atypical intestinal mycobacteriosis mimicking Whipple's disease (WD) in a HIV-infected patient diagnosed in vivo in Latin America. METHODS: Stool examination for parasites and culture were done. Jejunal biopsy was performed by Watson capsule and the fragment processed for histopathologic examination. Sections were also stained with special methods for enteropathogens. RESULTS: 38-yr-old HIV-infected homosexual man, presenting chronic diarrhea, fever, weight loss and esophageal candidosis was diagnosed to have atypical intestinal mycobacteriosis by jejunal biopsy. The sections showed a 2:1 villus/crypt ratio, regenerative changes in the deep epithelial cells and a moderate increase of inflammatory cells in the lamina propria. Some villi were enlarged by numerous foamy macrophages containing PAS-positive elongated rods mimicking WD. The differential diagnosis was done by Ziehl-Nielsen positive staining of bacilli. All stool tests were negative. CONCLUSIONS: Although uncommon, mycobacteriosis must be considered as a cause of diarrhea in HIV-infected patients and it should be searched for by stool and blood cultures and by jejunal biopsy.
