Injury: */1 Amputation of finger(s) (calculated by finger, i.e. tip = 1, hand = 5), crush,  fracture, laceration, pinch
In/Out: Hands in the die or out of the die operations
M/F: male/female operator
Cause: accidental tripping of foot pedal or hand control, failure or repeat of press, no safeguard in use (*/2 More than one cause or safety device)

MOST FREQUENT CAUSE OF INJURY IS ACCIDENTAL TRIP WHILE HANDS ARE IN DIE.
771 Fingers are missing since August of 1994

A request has been made to identify the states the reports are received from, the following is a list of the states: OH-28, WI-11, IL-10, MO-6, PA-6, TX-5, NY-4, NJ-4, TN-3, CT-3, RI, ALA, NE, GA, and MS all had 2. NC, OK, SC, AR, MD, MI, KS, IN and FLA all had one injury report.
