REVISED FCC FORM 159/159-C, REMITTANCE ADVICE & REMITTANCE ADVICE CONTINUATION SHEET



Effective September 15, 1997, a revised FCC Form 159 & 159-C, dated July 1997, will be made available for your use.

The FCC Form 159/159-C is a multi-use form which has been revised to include additional FCC information requirements, and to comply with the Debt Collection Improvement Act of 1996 (P.L. 104-134). The Act mandates that each Federal agency require each person doing business with that agency to furnish to that agency such person's taxpayer identifying number. The intent of this requirement is to use such numbers for purposes of collecting and reporting on any delinquent amounts arising out of such person's relationship with the Government. The Taxpayer Identification Number (TIN) may be either your Employer Identification Number (EIN) if you are a business applicant, or your Social Security Number (SSN) if you are an individual applicant.

The following points highlight the changes to the FCC Form 159/159-C. You must:

1) Enter in Block 1 the lockbox number of the service for which you are making the payment. The lockbox number is the six-digit P.O. Box number indicated in the appropriate Fee Filing Guide or Public Notice. The purpose of entering the lockbox number is to ensure that your filing is sent to the correct FCC Bureau/Office for processing.

2) Provide the payer's taxpayer identification number (TIN) in Section D, Block 25, and an applicant taxpayer identification number (TIN) in Section D, Block 26, if it is different for the payer. If you are paying for multiple applicants with a single FCC Form 159, each additional applicant must be listed separately in Section BB, of the FCC Form 159-C, Continuation Sheet, and each applicant must complete Section DD, Block 25, of the FCC Form 159-C.

3) Read and sign the Certification Statement, in Section E, Block 27. Your signature certifies that your fee payment on the Form 159 and supporting information are true and correct to the best of your knowledge.

4) Use your nine-digit taxpayer identification number (TIN) as your FCC Account Number. You can no longer use a ten-digit telephone number as your FCC Account Number.

Your taxpayer identification number (which will either be your EIN if you are a business or your SSN if you are an individual) will now be used as your FCC Account Number whether you pay by check, money order, credit card or wire.
