District HIV Care Co-ordinator, South Glamorgan Health Authority, Wales, UK.
ISSUE/PROBLEM: In 1989 service provision for people affected by HIV/AIDS in South Glamorgan, UK, was often very fragmented. There was very little networking or co-ordination. There was a clear need therefore for a Model of Care which would co-ordinate service provision for people affected by HIV/AIDS. DESCRIPTION OF PROJECT: Fifty-five people representing Health Authority, Social Services, and various other statutory and voluntary organisations and agencies attended a one-day workshop. The aims were: i) To bring together representatives who were currently, or who were likely to be involved in providing care and support to people affected by HIV/AIDS. ii) To present and describe the proposed Model of Care. iii) To provide a forum for discussion on how those representatives envisaged their agency working with the Model. iv) To identify any potential issues of concern that representatives had regarding the Model. RESULTS: One year later all the statutory and voluntary agencies are now committed to the Model of Care which ensures a co-ordinated programme of support for those affected and is a Model of good practice which gives a wider choice of services available to the consumer and greater reward to the service provider. LESSONS LEARNED: Quality of care is inextricably linked to: i) Commitment of all the relevant agencies ii) Good communication between the statutory and voluntary sector iii) Effective co-ordination.
