Double HIV1 and HIV2 seropositivity in Guinea.

3855 blood donors, 142 women from maternity and 218 patients (class III - IV) were screened between November 1987 and December 1988 for HIV1 and HIV2 antibodies in three sites in Guinea. Repeatedly reactive sera were confirmed in Paris with the Western blot technique. We attempted to discriminate between HIV1 and HIV2 reactivity in the sera that showed both HIV1 and HIV2 positivity in Western blot, using the new Peptilav 1-2 test (Diagnostic Pasteur) and the screening for anti Nef. - None of the 142 women were confirmed HIV positive. - 17 of 3855 donors (0,44%) were confirmed HIV positive: 7 to HIV1, 9 to HIV2 and one to the viruses. This serum was clearly discriminate as HIV2 with the Peptilav 1-2 test. - 35 of 218 patients were confirmed HIV positive: 17 to HIV1, 7 to HIV2 and 11 to both viruses with the Western blot. With Peptilav 1-2 we could discriminate only one of these sera as HIV1. It is also anti Nef positive; 4 sera gave stronger reactivity to HIV2; 3 of them could be screened for anti Nef and were negative; and 6 others showed simultaneous reactions to both virus peptides; 5 were anti Nef positive and 1 negative. This finding raises the possibility of true double infections or infection with a third new virus whose epitopes are shared by HIV1 and HIV2. These results indicate the presence of the two viruses in Guinea. The positive donors are predominantly infected with HIV2 (58,8%) and the patients with HIV1 (68,5%). 6 persons had serum reactivity suggesting simultaneous infection by HIV1 and HIV2 viruses or by another closely related virus. Only isolation of the viruses involved will bring a conclusion.
