Solicitation 51-SO-BC-8-00021 is issued as a Request For Quote. This solicitation and incorporated provisions and clauses are those in effect through FAC 2005-25. Acceptance at same location. Delivery on or before 07-25-08. Awardees will have to be registered at CCR.gov and complete a U.S. Census Bureau’s vender registration form. 4. Provide documentation of the following with your quote:

4a. Membership in the National Air Duct Cleaners Association (NADCA).

4b. Membership in the Indoor Air Quality Association (IAQA).

4c. One Air Systems Cleaning Specialist (ASCS) certified by NADCA on staff.

4d. One Certified Microbial Remediation Specialist (CMRS) certified by IAQA on staff.

4e. On site supervisor must be certified as an ASCS by NADCA.

4f. Business must be in operation minimum of five (5) years and submit records of HVAC systems cleaning.

4g. Licensing by the State of Indiana to perform work is not needed.

4h. Pollution Liability Insurance minimum of $1,000,000 per occurrence $2,000,000 per aggregate.









This amendment changes Census Solicitation Number 51-SO-BC-8-00021 as follows: The requirement for Pollution Liability Insurance minimum $1,000,000 per occurrence $2,000,000 per aggregate, is changed to General Liability Insurance in the amount of $2,000,000. Also a new document, which is now considered part of the Solicitation package, is attached containing the questions and Census responses from the pre-bid conference and site tour.
