48.1 Overview

This procedure requires the removal of hazards from laboratory spaces when the user plans to vacate laboratory space. These plans to vacate a lab may include:

49.4 Definitions

Decommissioning: The formal deactivation of a laboratory, assuring the safety of the space for further cleaning, renovation, or occupancy. The decommissioning process involves a survey by Safety, Health and Medical Services Division for those locations where chemicals and biologicals were used. Radiation Safety will check those locations where radioactive materials were used.

Laboratory: A facility where relatively small quantities of hazardous chemicals, biologicals, and/or radiological agents are used on a non-production basis.

49.5 Responsibilities

49.5.1 Directors, Division Chiefs, Branch Chiefs (and Supervisors)

49.5.2 Safety, Health and Medical Services Division (Code QH)

  1. All chemical and biological materials have been properly removed, disposed and/or stored, after it has been decided that the chemical should not be transferred to the ACE;
  2. All special in-house equipment that is no longer needed has been removed;
  3. Drawers and cabinets emptied, and fume hoods cleaned and/or decontaminated;
  4. All needles, razor blades and other sharps have been removed from cabinets, drawers and other surfaces; and placed in an approved sharps container for disposal;
  5. All counter tops have been properly cleaned and/or decontaminated.

49.5.3 Environmental Services (Code QE)

  1. A Hazardous Waste Accumulation Log label must be completed for each container removed from the lab. A "Form A" must be completed for the disposal of chemicals.
  2. Waste containers must be suitable for transport (container compatible with waste, tight fitting caps, outside of containers clean). Contact QE for containers.

49.5.4 Branch Chief/Supervisor

  1. These activities may include but are not limited to: decontamination of equipment, disposal of chemicals and cleaning lab spaces of old samples or other safety hazards.
  2. If an inspection is conducted, and abandoned chemicals are found with no routine inspection report, the Branch Chief will be held accountable for any fines NASA receives.
  3. The Branch Chief is responsible for verifying that materials in the departmental common spaces that belong to the PI have been removed. This includes locations where chemicals, biologicals, and radiologicals may be stored in freezers remote from the laboratory.

49.5.7 Principal Investigators (PI) shall:

  1. Arrange for the disposal of all unwanted chemicals by submitting a completed Form A, at least 14 days prior to the actual move date. It is recommended that unopened chemicals be transferred to the Ames Chemical Exchange (ACE) before considering disposal options.
  2. Ensure that all chemical shipments to off-site locations are packaged by trained individuals and sent in accordance with US Department of Transportation Requirements, (CFR Title 49).
  1. Notify the Radiation Safety Officer (RSO).
  2. Work with the RSO to survey and decontaminate all areas where radioactive materials were used or stored. Equipment in which radioactive materials were used/stored shall also be surveyed and decontaminated as necessary. The surveys shall be documented in the laboratory logbook.
  3. Transfer remaining radioactive materials to another permit holder or to the Radiation Safety Officer for disposal. Arrangements can be made, through the Radiation Safety Officer, to ship radioactive materials and radioactive equipment off site.
  4. Remove and destroy all labels indicating the presence of radioactivity.
  5. Reconcile the radioactive materials inventory in the logbook.
  6. Ensure that the RSO cancel dosimetry badges, if appropriate.
  1. Code QE shall be notified of compressed gas disposal prior to removing the gase(es).
  2. If gases are to be transferred to another PI, Code QE must be notified with the name of the person taking over responsibility of the gas(es) at least four weeks prior to transfer date.
  1. QH: For contractors to decontaminate biological safety cabinets, for information on biological, chemical, and physical hazard issues, and to schedule a decommissioning survey
  2. QE: For issues relating to the transfer of chemicals/gases and/or disposal of chemical waste (hazardous waste).
  3. c. Radiation Safety Office: For any issue involving radiological site evaluations and radioactive materials disposal.

Appendix A. Request for Room Laboratory Decommissioning Inspection

NASA Ames Research Center 

Safety, Health and Medical Services Division, Code QH, MUST verify 
decommissioning of all vacated laboratories. Decommissioning involves
inspection by QH and if applicable, by the RSO in locations of radioactive
materials use. All special in-house equipment must be removed; drawers and
cabinets emptied, and fume hoods cleaned by laboratory personnel. Radiation
Safety MUST be notified prior to transferring stock materials or possible
radionuclide contaminated laboratory equipment (centrifuges, glassware,
refrigerators, freezers etc.) to a new location. Radiation Safety MUST
conduct stock material transfers in most instances. Please indicate on this
form if a final waste pick-up by the Hazardous Waste Management Unit will be
required before vacating a laboratory.
