Objective: A cross sectional study of clinical and laboratory abnormalities in protease inhibitor (PI) treated patients (pts). Patients and Methods: All pts in the AIDS clinic receiving PI*3 m underwent clinical evaluation of 14 signs,10 anthropometric measures, testing for cholesterol (C), triglyceride (TG), apolipoprotein B/A1, 75gr 2hr OGGT, determination of insulin resistance (IR) by insulinemia or homeostasis model assessment. Results: 650 pts have been consecutively evaluated since July 98,results on 300 pts (m/f 83%/17%) are analyzed. Pts are aged 40 +/- 9 yrs,34% had an AIDS-defining disease,CD4=303 c/mm3 [1-1333],HIV RNA <500 cp/ml in 65% of pts, were on PI since 18 +/- 9 m. Major clinical signs were selected according to frequency: facial, limb,or buttock atrophy, venomegaly, increased abdominal wall thickness, neck, breast or waist size. Three clinical types were identified: lipoatrophic (71%), truncal pseudo-obesity (62%), mixed (46%). Of these,37%,39%,34% were non IR and non GI. Frequency of diabetes: 8.9%,9.1%,9.4%;glucose intolerance (GI):30.8%,27.8%,35.3%;IR without GI:22%,23%,20.9%;.hyper TG (>1.7 mmol/l) 34%,46%,36%;hyper C (>6.7 mmol/l) 76%,76%,71%;in the 3 types respectively. Median duration of PI was in the similar range (17-20 m) in the 3 clinical types. Median baseline minus current HIV RNA & CD4+ was identical in the 3 types. The frequency of diabetes, GI, and IR was lower (10.5%,7.9%,10.5%,respectively) in PI treated pts not incluable in the 3 clinical groups. Conclusion: Patients with PI>3 m develop a lipodystrophy syndrom comprised of 3 clinical types: lipoatrophy, pseudo-obesity or mixed. Diabetes mellitus, glucose intolerance or insulin resistance is a significant feature of this syndrom and is associated with some degree of dyslipidemia.
