ISSUES: Approx. 10,000 Indonesian Migrant Workers (IMW) going to/from Malaysia every month Participation in global action by protecting the cross-border of HIV Epidemic & keeping care of human rights. DESCRIPTION: In 1999 Ministry of Health of Malaysia licensed a medical check-up clinic for migrant workers named as "Persahabatan Medihelp" in Indonesia. This clinic has responsibility to do medical check-up for IMW at pre-departure by doing physical, chest X-Ray, urine, & blood tests including HIV testing by using ELISA kit. In August 2000 up to July 2001 Indonesian Healthy Life Movement Institute (LAHSI) did observance & intervention at this clinic purposed for human rights and policy development. LESSON LEARNED: Clinic is successful to filter IMW' health quality at pre-departure. Only IMW having fit certificate is acceptable to obtain working visa. Providing qualified migrant workers to both countries (skills & health). HIV testing with informed consent has not applied yet. After informing HIV+ status to patients they escaped from base-camp & missed. In the early of 2001 when human rights in health field becoming the most sensitive issues in Indonesia, HIV testing without informed consent become inappropriate. Understanding that HIV testing for migrant workers is discriminative, not effective, expensive & wasteful. In the other side we must be respectful to other countries health policy. RECOMMENDATIONS: Voluntary confidential counseling testing (VCCT) to migrant workers shall be applied in all clinics with overseas license. This VCCT shall be done with informed consent at pre-test & post-test counseling, & keeping confidentiality. Empowering those are with HIV+ status to work in the area of HIV/AIDS prevention, care, and treatment campaigns.
