Direct costs for percutaneous transluminal angioplasty with and without stent implantation: a comparison.

OBJECTIVE: Percutaneous transluminal angioplasty (PTCA) is effective in treating angina pectoris. Stent implantation allows a reduction in the incidence of restenosis, but is more expensive. We wanted to compare the direct medical costs of the 2 procedures at our institution. METHODS: One hundred-and-one consecutive patients hospitalized between May 27 and November 24, 1997 for PTCA because of angina were included. Stent implantation was decided by the cardiologist at the time of the procedure. Direct medical costs for the procedure were computed for each patient. RESULTS: Thirty-nine patients were treated by PTCA alone, and 62 with PTCA and stent. The 2 groups were similar as far as age, gender, and coronary risk factors were concerned. Conclusions: PTCA with stent implantation is twice as expensive as PTCA alone. The major part of the difference in costs is linked with material acquisition cost (95%). This difference in costs could be reduced by material price negotiation and partially compensated by a reduction in the number of secondary prodecures. This hypothesis must still be verified in the Swiss heatlh care system.
