High complication rate of herpes zoster (HZ) at an outpatient HIV clinic.

To determine the incidence and clinical manifestations of HZ in an hospital-based HIV clinic, we reviewed the records of all patients diagnosed with HZ at or after their first clinic visit. 45 cases of HZ occurred during 1614 person-years of follow-up for an incidence of 2.8 cases per 100 person-years. Complication of HZ occurred in 18 (40%) patients: bacterial superinfection (n=6); ocular complications (n=5) including conjunctivitis, uveitis, keratitis, retinitis, exposure keratopathy); neurologic complications (n=4); including aseptic meningitis [n=2], radiculitis, myelitis); atypical chronic skin lesions (n=5); and postherpetic neuralgia (n= 6). Compared to those with uncomplicated HZ, those with complicated HZ were more likely to have been on antiretroviral therapy (83% vs. 44%; p=.013) and had lower median CD4 cell counts (68 vs. 220 cells/mm3; p=.11). By multivariate analysis, only antiretroviral use was associated with an increased risk of complication of HZ (odds ratio 6.25; 95% CI 2.34-16.7; p=.013). Thus, we observed a high incidence and wide spectrum of complications of HZ at an hospital-based HIV clinic.
