Aqueous Humor Penetration of Linezolid after Intravenous Administration.

BACKGROUND: Linezolid (LNZ) is an antibiotic active against Gram positive microorganisms, main cause of traumatic, postoperative or bleb-related endophthalmitis. Our objective is to analyse diffusion of LNZ into aqueous humor (AH) after intravenous administration. METHODS: Clinical trial including 21 patients who underwent cataract extraction from December 2002 to February 2003. Cataract removal was performed at varying times (from 60 to 270 minutes) after the end of a 20 minutes infusion of LNZ. In each patient, serum samples were obtained 1 hour after the end of LNZ administration (C[max)]; AH and a second serum sample were simultaneously taken during the operation (C[AH] and C[2] respectively). Concentrations of LNZ were determined by high-performance liquid chromatography assay. RESULTS: Out of 21 patients, 10 were men (47.6%) and 11 women (52.4%). Mean age was 78 years (range 64-87; SD 6.04). Mean patients weight was 71.65 kg (range 58-91; SD 9.3). Mean serum creatinine was 1.13 mg/dl (range 0.9-1.4; SD 0.16). Mean peak serum concentration (C[max)] of LNZ was 13.49 mcg/mL (range: 10.25-18.04; SD 2.41). Mean C[AH] at the first hour following LNZ administration was 4.94 mcg/mL (range 2.68-6.12; SD 1.03) and mean ratio (R = C[AH]/C[2)] was 0.43 (range 0.23-0.52; SD 0.09). Mean C[AH] at 135 to 270 minutes after LNZ administration was 5.17 mcg/mL (range 4-6.96; SD 0.95) and mean R was 0.48 (range 0.34-0.66; SD 0.11). All patients had C[AH >] 4 mcg/mL 2 hours after LNZ administration. CONCLUSIONS: Two hours after 600 mg intravenous administration of LNZ, its mean concentration in AH reach MIC90 of Staphylococcus aureus and Staphylococcus epidermidis.
