This is a resend as I did not strip the format information on the last one.

Here is the final DC-8 seat allocation as presently planned. AS you can see
below, I have dropped off the DC-8 and will travel commercial. This gives
as many seats as possible to the investigators. As presently planned, I
will not go to the Azores, but will travel directly from Shannon to Bangor.

Also attached is the NASA policy regarding Life and Health Insurance while
flying on NASA Aircraft. If you think you may fly on the DC-8, please give
this information careful attention.

INSURANCE COVERAGE WHILE ON NASA AIRCRAFT

LIFE INSURANCE
Life insurance is the responsibility of the individual. NASA cannot and
does not provide free life insurance for anyone aboard NASA aircraft.

Some private life insurance policies may exclude coverage while on
non-commercial aircraft or while serving as a member of a flight crew.  The
only way to be certain of the coverage of a given policy is to contact the
insurance agent responsible for the policy.

NASA does, however, make some low-cost life insurance policies available
for purchase by its employees.  The information provided below is general
in nature and should not be construed as a definitive explanation of the
coverage of any policy.  Detailed information can be obtained from the
Employee Benefits and Records Branch.

Federal Employees Group Life Insurance (FEGLI):
Available to virtually all federal employees.  Paid by employee through
payroll deductions.  Covers members while on any NASA aircraft, regardless
of their status as passengers, pilots, or other crew members.  Basic
coverage is approximately one year's salary plus $2,000.  Other higher
amounts are also available at additional cost.

NASA Employees Benefits Association (NEBA) Group Insurance: Available to
virtually all NASA employees.  Paid quarterly by employee.  Covers members
while on any NASA aircraft, regardless of their status as passengers,
pilots or other crew members.  Basic coverage is determined by employee's
salary. Other higher amounts are also available at additional cost.
	The basic plan provides double payment for most
	accidental deaths, but does not provide this double
	coverage for death on non-commercial aircraft (such
	as NASA aircraft).

NEBA also makes available a special Travel Accident Insurance that may be
purchased either in addition to the basic coverage or as a separate policy.
The employee can choose among four	 versions and four coverage levels
ranging from $25,000 to $100,000.
	I.   All personal travel by car and/or commercial air,
	     sea, and land carriers.  And all local travel while
		on NASA business.
	II.  A11 travel (including as a passenger on NASA
		aircraft)
	III. Same as II but includes coverage as a pilot or crew
		member of NASA administrative or support
		aircraft.
	IV.  Same as III but includes pilots and crew
		members while on NASA proficiency flights.

HEALTH INSURANCE
Health insurance is the responsibility of the individual. NASA cannot and
does not provide free health insurance. There are, however, numerous health
insurance plans available to virtually all federal employees. Payment is
made by payroll deduction. Employees planning travel in foreign countries
should consult their health plan's brochure for claims information.

WORKER'S COMPENSATION (OWCP)
While technically not an insurance, the Office of Worker's Compensation
Programs in the U.S. Department of Labor (415-556-5757) provides certain
benefits to federal employees who are injured or killed In the performance
of any of their official duties.

	Medical expenses are usually billed directly to the OWCP
	through the use of a form called a CA-l6 (available at
	the Ames Employee Benefits and Records Branch). In some
	foreign locations the CA-16 will not be honored. In
	this case the employee may have to cover the costs
	personally and be later reimbursed by the OWCP. (For
	assistance in a foreign country contact the San
	Francisco OWCP office number listed above or the local
	American Consulate).

	100% compensation for lost wages is provided NASA for up
	to 45 days after a traumatic injury. For longer periods
	of disability (or non-traumatic injuries), the OWCP
	provides a continuing compensation payment of between
	66% and 75% of salary.

	Death benefits include a nominal amount for burial
	expenses and a continuing payment of 50% of salary to
	the widow (or widower). If there are dependent children,
	the spouse's amount is reduced to 45% and 15% is added
	for each child (to a maximum total of 75%).

NON-NASA EMPLOYEES
NASA does not provide any insurance protection for non-NASA
employees.  NASA officials inviting such personnel to fly on
NASA operated aircraft should advise their invitees to
contact their own insurance carrier or their own employer to
determine what insurance coverage, if any, may be available
to them.
