Background: Since years we know, that moderate and regularly sport activity has a positive effect on the quality of life. Earlier investigations with HIV-1 people show mixed results, that an exercise program combining aerobic and progressive resistance training provides synergistic benefits for the cardiovascular, muscle skeletal and immune systems. It is unclear if sports has a positive effect on the psychological well-being and functions as a buffer against the stress. Methods: Participants in our study are adults of both sexes, in different stages of HIV-infection and with different medical treatments. The sport program take 4 months and we measure psycho-social (quality of life, coping strategies, locus of control, psychopathology) and physiological parameters (sport medical conditions and immune status) before, at the end of the intervention and four months after. Results: At the end of the sport intervention 54% of the participants showed active and adaptive coping strategies and a adequate balance between their internal and external resources and could cope better with their anxiety and depressive feelings. In general 69% indicated an improvement of their quality of life, that means an increase in their social contact, corporal resources, and a reduction of illness and pains. 36% of the participants showed a significantly increase in the number of CD 4 cells and 72% had a virus load which is not measurable. 77% of the sample have taken HAART. Discussion: In general the effects of the sport program on the quality of life were very positive however we observed a mediating impact of compliance, medication and stadiums of HIV infection on our results. The social support also played an important role. As a consequence , we offer now a sport program, to assess the long-term effects of exercise on immunological, physiological and psychological variables.
