Introduction: The EQ-5D and HUI are available for empirical utility estimation in clinical practice, but no empirical information is available on the relative performance of the two measures. This study empirically evaluated the EQ-5D and HUI for description and valuation of health-related quality of life in children with sequelae after bacterial meningitis. Methods: Seven standardised descriptions of children with permanent sequelae after bacterial meningitis were classified on the EQ-5D and the HUI-15Q classification questionnaires by a panel of 28 paediatricians. The descriptions were associated with 1) deafness, 2) mild hearing loss, 3) epilepsy, 4) mild mental retardation, 5) severe mental retardation combined with tetraplegia, 6) paresis of the leg and 7) mild mental retardation combined with epilepsy and paresis of the leg. Utility scores were calculated using EQ-5D and HUI standard scoring algorithms. Absolute utilities for the health state descriptions and their ranking order were compared. Results: HUI utilities were substantially lower than the EQ-5D for all health states (mean difference of 0.11, 95% CI: 0.08-0.14). The EQ-5D and HUI resulted in different absolute utility scores, especially for states associated with deafness and mental retardation. The HUI2 and HUI3 resulted in similar ranking orders but the EQ-5D inverted the ranking order of two states on two occasions. Mean agreement on the ranking order between EQ-5D and HUI was 0.90. Both methods were highly reliable. Conclusions: Although good reliability for all, the EQ-5D and HUI result in different absolute utility scores. The HUI may be preferable to EQ-5D in studies where 'sensation' (hearing, vision, speech) and / or 'cognition' are important dimensions. Furthermore, sensitivity analysis to utility scores is generally recommended, because these figures may not be so absolute as sometimes seems to be thought.
