U.S. Passports

  • As of February 1, 2008 the age requirement for two parent consent for minor passport applications will be raised from 14 to 16 years.

Obtaining and Renewing U.S. Passports

All routine U.S. passport requests are processed and printed at the New Orleans Passport Agency in Louisiana. Passport renewals normally can be done in about 5 working days.

Important Passport Security Enhancement - No Amendments

Effective September 26, 2005, as an additional security enhancement, Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports.  

If the change is within one year after issuance, they will submit a U.S. Passport Re-Application Form (DS 5504), together with the passport that needs the change, the documentation required, and new passport pictures. There will be no charge for routine processing.

If the change is more than one year after issuance, customers will need to submit an Application for Passport By Mail (DS-82), together with the passport that needs the change, the documentation required, and new pictures. The cost includes the $55 application fee and $20 security surcharge, for a total of $75 for routine processing. Of course, if the requested data change is due to a printing error by Passport Services, there will be no charge.

Those only needing to add visa pages to their passports will submit an Application for Additional Visa Pages (DS-4085). This service is always free of charge when routine.

All of these forms will be available online at travel.state.gov. The Passport Amendment/Validation Form (DS-19), previously used to amend passports, will no longer be valid. For more information on applying for a U.S. passport, forms, and a wealth of other international travel information, please visit the web site at travel.state.gov.

To Obtain or Renew a U.S. Passport, Please Submit:

  • Your most recent U.S. passport; or
  • If you do not have a U.S. passport, proof of U.S. citizenship (e.g. a certified original U.S. birth certificate or a Certificate of Naturalization, and a valid, current picture ID (e.g. a Hungarian ID card);
  • Two (2) recent passport photographs (5cm x 5cm or 2"x2") taken of the full face with both ears clearly visible, with a light background; and
  • Fees payable in cash (USD or HUF), travelers checks, money orders or credit cards (Visa, MasterCard, American Express, Diners Club, and Discover).

Passport Application for Children

On November 19, 2007 a revision of 22 Code of Federal Regulations (CFR) part 51 regarding passports was published in final in the Federal Register with an effective date of February 1, 2008.

Effective February 1, 2008, passport applicants who are U.S. citizen minors under the age of 16 must appear personally with their parents to establish identity, proof of citizenship and proof of relationship.  Previous U.S. passports are not acceptable as proof of minor's relationship to the parent(s)/guardian(s) (but should be submitted with the application per 22 CFR 51.8).

1. You will need to submit one of the following to establish the parental relationship/guardianship:

  • Certified U.S. Birth Certificate (with parents' names); or
  • Certified Foreign Birth Certificate (with parents' names and translation, if necessary); or
  • Report Of Birth Abroad (Form Fs-240) (with parents' names); or
  • Certification Of Birth Abroad (Form Ds-1350) (with parents' names); or
  • Adoption Decree (with adopting parents' names); or
  • Court Order establishing custody; or
  • Court Order establishing guardianship.

If the parent(s)'/guardian's name(s) is/are other than that on these documents, evidence of legal name change is required.

2. Each parent or guardian must submit one of the following:

  • Valid Drivers License; or
  • Valid Official U.S. Military ID; or
  • Valid U.S. Government ID; or
  • Valid U.S. or Foreign Passport with recognizable photo; or
  • Naturalization/Citizenship Certificate from USCIS with recognizable photo; or
  • Alien Resident Card from USCIS.

3. Both parents must appear together with the child or

  • One parent may appear and sign if that parent submits the second parent's notarized statement of consent authorizing passport issuance for the child.  The form of the consent may be either a notarized DS-3053 "Statement Of Consent:  Issuance of a Passport to a Minor" (currently being revised to reflect that the requirement pertains to minors under the age of 16) or a notarized written statement with the same information on a sheet of paper from the non-appearing parent, or

  • One parent may appear and sign if that parent also submits primary evidence of sole authority to apply, such as one of the following:
    -- child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or
    -- consular report of birth abroad (form FS-240) or certification of birth abroad (form ds-1350) listing only applying parent; or 
    -- a court order granting sole custody to the applying parent (unless child's travel or passport application is restricted by that order); or
    -- an adoption decree (if applying parent is the sole adopting parent); or
    -- a court order specifically permitting applying parent's or guardian's travel with the child or passport application; or
    -- a judicial declaration of incompetence of non-applying parent; or
    -- a death certificate of the non-applying parent.

  • If none of the above documentation is available, the applying parent/guardian should submit a form DS-3053 Statement of Consent: Issuance of a Passport to a Minor (being revised as noted above to reflect that this requirement pertains to minors under the age of 16).

4. A third-party in loco parentis applying on behalf of a minor under the age of 16 must submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. When the statement or affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

Application Forms

The forms can be downloaded from the U.S. Department of State web site:

DS-11 if

  1. You have never had a U.S. passport.
  2. You are under age 16.
  3. Your most recent passport was issued more than 15 years ago.
  4. Your passport was lost or stolen. If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64.

DS-82 (Renewals Only)

This form should be used for renewing a previously issued and still valid U.S. passport in person, if

  1. You can submit your most recent passport AND
  2. You were at least 16 years old when your most recent passport was issued AND
  3. You were issued your most recent passport in the last 15 years AND
  4. You use the same name as on your most recent passport or since your passport was issued, you have had your name changed by marriage or court order and can submit proper documentation to reflect your name-change.

If you do not meet all 4 of the above requirements, you cannot use Form DS-82! You must apply on a DS-11.

DS-5504 
 
This form is used within one year of passport issuance for name-changes and any other descriptive information changes, as well as for the replacement of limited passports and correction of any printing errors. The DS-5504 must be submitted with the passport to be replaced, the documentation required for the change, and new photos. There is no charge for routine processing of these changes. After one year of the passport issuance date, Form DS- 82, Application for Passport by Mail, or Form DS-11, U.S. Passport Application, as appropriate, must be submitted and all applicable fees paid.

DS-4085

This form is only used for adding more visa pages to valid passports. There is no charge for routine processing.

DS-3053

Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor under the age of 16. For details please see Passport Application for Children.

Lost or Stolen Passports

If you lose your passport, a replacement passport can be issued upon presentation of proof of identity and U.S. citizenship (i.e. birth certificate, driver's license, or previous passport). In addition to the ID, two 5 cm x 5 cm (or 2"x2") photos with a light background will be required. If the passport has been stolen, first report the theft to the local police and obtain a police report for presentation to the consular officer.

Passport Fees

All fees are payable in U.S. dollars (USD) in cash, travelers checks, money orders or credit cards (Visa, MasterCard, Diners Club, American Express, and Discover), or the equivalent in Hungarian Forints (HUF). We do not accept personal checks.

  • A routine renewal of a ten (10) year adult passport is 75 USD;
  • A routine renewal of a five (5) year minor passport (for those under age 16) is 85 USD;
  • A lost, stolen or mutilated passport replacement is 100 USD for adults and 85 USD for minors;
  • A first-time adult passport is 100 USD and a first-time minor passport is 85 USD.

Passport Photographs

Submit two identical color photographs taken within the last 6 months, 2x2 inches (5x5 cm) in size, with an image size from bottom of chin to top of head (including hair) of between 1 and 1 3/8 inches (2.5 cm - 3.5 cm). Photographs must be clear, front view, full face, taken in normal street attire without a hat or dark glasses, and printed on thin paper with a plain light (white or off-white) background. Photographs retouched so that your appearance is changed are unacceptable.

Unable to Come to the Consulate?

If you are not physically able to come to the Consulate, you can obtain a power of attorney for someone to act on your behalf to bring to the Consulate your completed application with photos and the required passport fees. Please note that this procedure does not apply to lost/stolen/mutilated passport cases, first-time applicants, or to minor children's applications.

Courier Service to Deliver Passports

The Embassy contracted with a private courier service for overnight delivery of passports anywhere in Hungary for a fee of HUF 1,540. If you are interested in using the courier service, please speak with a member of the ACS staff.
