A new method for identifying control patients for prospective studies: a population based survey.

PROBLEM AND OBJECTIVES: To validate a new method of identifying control patients for prospective longitudinal comparison of treatment, we conducted population-based telephone surveys in New York and Montreal. DATA AND METHODS: Using voting lists in Montreal and Medicare beneficiary data in New York City, patients with hip and knee pain were identified by telephone survey. The severity of symptoms was rated using the Hip Rating Questionnaire (HRQ) or the Knee Rating Questionnaire (KRQ). To validate the methodology, patients with a range of HRQ and KRQ scores resulting from their self-reports of hip or knee pain on the surveys, were x-rayed and evaluated by rheumatologists and orthopedists in New York and Montreal. RESULTS AND CONCLUSIONS: Intra-articular disease was confirmed in 75% of the patients who presented for x-rays and physical examinations. Among people with hip pain in our telephone survey, the mean HRQ score was 63+/-8.7 in New York and 57.7+/-8.7 in Montreal. Among those with knee pain, the median score was 60.0+/-10.8 in Montreal and 65.2+/-8.5 in New York. These scores correspond to the median pre-operative HRQ scores of 60 found in a cohort of hip arthroplasty patients. The distribution of scores were similar in Montreal and New York City. The severity of hip pain was also comparable in the African American, Latino, and White communities in New York City. IMPLICATIONS FOR AUDIENCE: Patients with hip and knee pain identified by a community survey have severity of disease comparable to surgically treated patients. Such findings demonstrate the viability of utilizing population-based telephone surveys to identify and recruit a population of control patients for prospective studies.
