OBJECTIVE: To determine if Pneumocystis carinii pneumonia (PCP) prophylaxis impedes the detection of Pneumocystis cysts in serial induced sputums. METHODS: Review of charts of 61 patients diagnosed with PCP between 1/31/1981-2/2/1990. 8 patients were excluded because no sputum samples (6), or alternative diagnosis made (2). 51 episodes of PCP occurred in 47 patients (39 proven, 12 presumed using strict CDC criteria). Further 6 patients had probable PCP (p O2 greater than 70 mmHg). 5 patients were classified as having no prophylaxis because they received less than one month's prophylaxis (3), or were noncompliant (2). Positivity for cysts of Pneumocystis in either first, second, or third induced sputums, or subsequent bronchoscopy was noted. RESULTS: TABULAR DATA, SEE ABSTRACT VOLUME. Of the 18 proven episodes on prophylaxis, 17 were positive on first induced sputum (94%), one was positive on second induced sputum (6%). Of the 21 proven episodes without prophylaxis 10 were positive on first induced sputum (47%), 6 on second (28%), 3 on the third (14%), one on the fifth (5%) and one at bronchoscopy following 3 negative sputums (3.5%). CONCLUSION: PCP prophylaxis does not make the detection of Pneumocystis cysts more difficult using serial induced sputums as a method of diagnosis. Patients on PCP prophylaxis are not more likely to be diagnosed presumptively than those not receiving prophylaxis.
