ERRORS IN THE NHEFS DATA TAPES AND SURVEY DIFFERENCES
The NHEFS Public Use data tapes have been subjected to a great deal of careful editing.
However, due to the large volume of data in the series, it is likely that a small number of errors or
discrepancies remain undetected.
In general, the NHEFS data management team has not attempted to resolve substantive data
discrepancies that may exist 1) within the 1992 NHEFS data tapes, or 2) between the 1992
NHEFS data tapes and the data tapes of the original National Health and Nutrition Examination
Survey (NHANES I) and/or other NHEFS followup waves.
Introduction to the 1992 NHEFS Vital and Tracing Status Data Tape Documentation
The NHANES I Epidemiologic Followup Study (NHEFS) is a longitudinal study which uses as
its baseline those adult persons ages 25 to 74 years who were examined in the first National
Health and Nutrition Examination Survey (NHANES I). The NHEFS is comprised of a series of
four followup surveys. The first wave of data collection, the 1982-84 NHEFS, was conducted
from 1982 to 1984 and included all persons who were between 25 and 74 years at their NHANES
I examination (n=14,407). The second wave of data collection, the 1986 NHEFS, was conducted
for members of the NHEFS cohort who were 55-74 years at their baseline examination and not
known to be deceased at the 1982-84 NHEFS (n=3,980). The third wave, the 1987 NHEFS, was
conducted for the entire non-deceased NHEFS cohort (n=11,750). The fourth wave of data
collection, the 1992 NHEFS, includes the entire non-deceased NHEFS cohort (n=11,195).
NHANES I collected data from a national probability sample of the United States civilian
noninstitutionalized population between the ages of 1 and 74 years. The survey, which included
a standardized medical examination and questionnaires that covered various health-related
topics, took place from 1971 through 1974 and was augmented by an additional national sample
in 1974-75. The NHANES I sample included 20,729 persons 25 to 74 years of age, of whom
14,407 (70 percent) completed a medical examination. The design, content and operation of
NHANES I has been described elsewhere (Vital and Health Statistics, Series 1, Nos. 10a, 10b,
and 14).
Although NHANES I provided a wealth of information on the prevalence of health conditions
and risk factors, the cross-sectional nature of the original survey limits its usefulness for studying
the effects of clinical, environmental, and behavioral factors and in tracing the natural history of
disease. Therefore, the NHEFS was designed to investigate the association between factors
measured at the baseline and the development of specific health conditions. It originated as a
joint project between the National Center for Health Statistics (NCHS) and the National Institute
on Aging with collaboration from components of the National Institutes of Health and other
Public Health Service agencies. The 14,407 participants who were 25 to 74 years of age when
they were examined in NHANES I (1971-75) were included in the followup study population.
In the first wave, the 1982-84 NHEFS, data were collected on all 14,407 subjects (i.e.,
individuals examined at NHANES I) in the cohort. Tracing of subjects began in 1981 and data
collection was conducted from 1982 to 1984. Approximately 93 percent (n=13,383) of the
cohort was successfully traced by the end of the survey period. Detailed information on the
design, content, and operation of the 1982-84 NHEFS may be found in the Plan and Operation of
the NHANES I Epidemiologic Followup Study 1982-84, Vital and Health Statistics, Series 1,
No. 22. The basic design of the 1982-84 NHEFS consisted of the following components:
-- tracing subjects or their proxies to a current address;
-- acquiring death certificates for deceased subjects;
-- performing in-depth interviews with the subjects or with their proxies including,
for surviving subjects, taking pulse, blood pressure, and weight measurements of
subjects; and,
-- obtaining hospital and nursing home records, including pathology reports and
electrocardiograms.
The second wave of NHEFS, the 1986 NHEFS, was conducted to assess changes in the health
and functional status of the oldest members of the NHEFS cohort since the last contact period. It
included 5,677 subjects who were 55 years or older at their NHANES I examination (almost 40
percent of the entire NHEFS cohort). Data collection was restricted to 3,980 subjects aged 55
years or older at NHANES I who were not known to be deceased at the time of the 1982-84
NHEFS, regardless of their tracing or interview status in 1982-84. The remaining 1,697 subjects
who were deceased at the time of the 1982-84 NHEFS were excluded from additional data
collection in the 1986 NHEFS. Detailed information on the design, content, and operation of the
1986 NHEFS may be found in the Plan and Operation of the NHANES I Epidemiologic
Followup Study, 1986, Vital and Health Statistics, Series 1, No. 25.
The 1987 NHEFS, the third wave of data collection, was conducted to continue to collect
information on changes in the health and functional status of the NHEFS cohort since the last
contact period. Tracing and data collection were conducted during this followup survey only for
the members of the NHEFS cohort who had not been identified as deceased in 1982-84 or 1986
(n=11,750) regardless of their previous tracing or interview status. The 2,657 previously
deceased subjects were excluded from further data collection in the 1987 NHEFS. Detailed
information on the design, content, and operation of the 1987 NHEFS may be found in the Plan
and Operation of the NHANES I Epidemiologic Followup Study, 1987, Vital and Health
Statistics, Series 1, No. 27.
The fourth wave of the NHEFS, the 1992 NHEFS, collected information on changes in the health
and functional status of the NHEFS cohort since the last contact. The 1992 NHEFS cohort
includes 3,212 subjects who were deceased at the time of the 1982-84, 1986 and 1987 NHEFS
and 11,195 subjects who were not known to be deceased prior to the 1992 NHEFS data
collection period. Tracing and data collection in the 1992 NHEFS was undertaken only for the
non-deceased subjects, regardless of their tracing or interview status in previous surveys. The
3,212 previously deceased subjects were excluded from additional data collection in the 1992
NHEFS. Detailed information on the design, content, and operation of the 1992 NHEFS may be
found in the Plan and Operation of the NHANES I Epidemiologic Followup Study, 1992, Vital
and Health Statistics, Series 1, No. 35.
The design and data collection procedures adopted in the 1992 NHEFS were very similar to the
ones developed in the previous surveys: subjects were traced; subject and proxy interviews were
conducted; and, health care facility abstracts and death certificates were collected. All subjects
whose vital status was not obtained through tracing procedures were considered lost-to-followup
in the 1992 NHEFS. In some cases, information about the death of a subject was obtained from a
former neighbor, a relative or another tracing source. Although this information was noted in the
subject's tracing record, he or she was considered lost-to-followup unless the information was
verified by means of a death certificate or proxy interview. A subject's death had to be confirmed
by either a death certificate or proxy interview.
In addition to verifying the subject's vital status, the tracing process also was used to obtain the
current address of surviving subjects as well as to identify a knowledgeable proxy respondent for
deceased subjects. Respondents who were identified and located through the tracing procedures
were asked to participate in a telephone interview. In a few cases, subjects who had been traced
successfully could not be relocated for the interview. Only their vital status and the date when
they were last traced in the 1992 survey are available.
A major difference between the NHEFS waves was the manner in which the interviews were
conducted. In the 1982-84 NHEFS, the two-hour subject interview was usually conducted in-
person while, in subsequent followups, the interview was shortened to approximately 30 minutes
in length and was conducted primarily by telephone. In addition, since the questionnaire was not
administered in-person, no physical measurements were made in the later followups.
The 1992 NHEFS interviews were conducted over the telephone using a Computer Assisted
Telephone Interviewing (CATI) system. CATI is a telephone interviewing technique that allows
the interviewer to enter the answers supplied by the respondent directly into the computer. Thus,
editing and coding time is reduced and keypunching from a hard copy questionnaire is
eliminated. A computer program drives the questionnaire so that the correct skip patterns are
followed and the appropriate questions are displayed on the computer monitor. The skip patterns
are based on information gathered from previous data collection waves or on responses provided
during the 1992 interview. For example, certain questions on pregnancy and menstrual history in
the 1992 interview were programmed to be skipped automatically if the subject was male or if
the female subject was interviewed previously and was 45 years of age or older at the time of that
interview. Edit and logic checks are incorporated into the data collection system itself, thus
improving the quality of the data.
As of July 19, 1993, the end of the 1992 NHEFS data collection period, 10,079 (90.0 percent) of
the 11,195 members of the 1992 NHEFS cohort had been successfully traced. Interviews were
conducted for 9,281 subjects (92.1 percent of those successfully traced). In addition, 10,535
facility stay records were collected for 4,162 subjects using information obtained from the
interview, death certificate, or some other source. Death certificates were obtained for 1,374
(98.7 percent) of the 1,392 subjects who were known to have died since last contact. Fifteen
additional death certificates were collected for subjects who were previously identified as
deceased by proxy interview only. Detailed information on the design, content, and operation of
the 1992 NHEFS may be found in the Plan and Operation of the NHANES I Epidemiologic
Followup Study 1992, Vital and Health Statistics, Series 1, No. 35.
The data collected from the 1992 NHEFS are stored on four separate tapes:
1) Interview tape -- contains the data collected from the subject and proxy
interviews,
2) Health Care Facility Stay tape -- contains information on reports of stays in
hospitals and nursing homes as well as information abstracted from facility
medical records,
3) Mortality Data tape -- contains data abstracted from the death certificates from all
three NHEFS surveys.
4) Vital and Tracing Status tape -- contains summary information about the status of
the entire NHEFS cohort. This tape is discussed below in more detail.
The Vital and Tracing Status tape is a master file containing vital status, tracing, and
demographic data for all NHEFS subjects (N=14,407), even if they were identified as previously
deceased at the time of the 1992 NHEFS. In addition, it provides users with information on the
availability of different survey components for each of the four followup periods for each subject.
For example, variables are contained in the file to indicate whether or not an interview was
completed or an abstract of an overnight hospital stay was obtained for a given survey period.
Positions 12-16 contain the Sample Sequence Number, which is the subject identification
number. This number is unique for each subject and is used when linking data tapes. Thus, the
1992 NHEFS Vital and Tracing Status tape can be linked to all other NHEFS and NHANES I
data tapes.
The 1992 NHEFS Vital and Tracing Status file has been organized into seven main sections:
1) Positions 17-24 contain the most recent information on the vital status and date
last known alive for each subject. Data collected in the 1992 NHEFS were used
to update these variables unless the subject was identified as deceased in a
previous survey period or the subject could not be located in the 1992 Survey. In
those two cases, information from the previous data collection period was
retained.
2) Positions 25-49 contain background information for the subject, such as age at
NHANES I exam (positions 39-40), sex (position 46), and race (position 47).
3) Positions 52-68 store selected mortality data for all deceased subjects. This
section includes information such as date of death (positions 61-66), underlying
cause of death (positions 53-56) and whether death certificate information is
available on the 1992 NHEFS Mortality Data Public Use tape (position 52).
4) Positions 102-126 contain summary information on the data collected in the 1982-
84 NHEFS. This section also provides users with information on the availability
of the different components for each subject in the 1982-84 Survey. For example,
variables are contained in this section to indicate which type of questionnaire was
administered during the interview and whether the subject has any hospital or
nursing home stay records on the revised 1982-84 NHEFS Health Care Facility
Stay Public Use data tape.
5) Positions 127-157 contain summary information on the data collected in the 1986
NHEFS, including the availability of different survey components for each
subject. The layout of this section is essentially identical to the layout of the
fourth section above which stores the NHEFS 1982-84 Followup summary data.
Variables in this fifth section are coded 9 or 99 (inapplicable), depending on the
length of the variable field, for the 1,697 subjects who were known to be deceased
at the NHEFS 1982-84 Followup. In addition, the variables in this section are
coded 8 or 98 for the 8,730 subjects who were less than 55 years at baseline
examination and therefore not included in the 1986 NHEFS cohort.
6) Positions 158-187 contain summary information on the data collected in the 1987
NHEFS, including the availability of different survey components for each
subject. The layout of this section is similar to the layout of sections four and
five. Variables in this section are coded 9 or 99 (inapplicable), depending on the
length of the variable field, for the 2,657 subjects who were known to be deceased
in the 1982-84 or 1986 NHEFS.
7) Positions 188-218 contain summary information on the data collected in the 1992
NHEFS, including the availability of different survey components for each
subject. The layout of this section is similar to the layout of sections four, five,
and six. Variables in this section are coded 9 or 99 (inapplicable), depending on
the length of the variable field, for the 3,212 subjects who were known to be
deceased in the 1982-84, 1986, or 1987 NHEFS.
8) Positions 219-272 contain additional demographic data available from the
NHANES I and NHEFS such as the NHANES I sample weights (positions 220-
255), NHANES I subsamples (position 219), NHANES I revised pseudo strata
and primary sampling numbers (positions 256-261), NHANES I poverty area
indicator (position 262), NHANES I state residence (positions 263-264), and state
of residence for each NHEFS survey period (positions 265-272).
Because of differences in the complexity of the 1982-84, 1986, 1987, and 1992 waves and/or
changes in data collection procedures, the availability and coding of data in the various survey
sections varies across survey years. For example, in the 1982-84 NHEFS, a mail questionnaire
was considered as a non-response to the interview component since not enough information was
obtained from it to code onto the interview file. Since the 1986 NHEFS, however, a more
detailed mail questionnaire was developed which provided enough information to have a mail
response coded onto the subsequent NHEFS Interview tapes.
Finally, please note that the original 1982-84 NHEFS Health Care Facility Record Public Use
file, released in September 1987, contained information only from facility abstracts. However,
this 1982-84 file has since been revised and re-released. It now has the same format as the 1986,
1987, and 1992 NHEFS Health Care Facility Stay Public Use files. The 1982-84 NHEFS facility
stay variables in positions 124-125 on the 1992 NHEFS Vital and Tracing Status Public Use data
tape are based on the revised 1982-84 NHEFS Health Care Facility Stay Public Use tape.

DEMOGRAPHIC AND TRACING INFORMATION
17 Current Vital Status
1 = Alive
3 = Dead
4 = Unknown
5 = Traced alive with direct subject contact
6 = Traced alive without direct subject contact, subject's vital status
verified
NOTE: The Current Vital Status (position 17) and the associated Date Last
Known Alive (positions 18-23) are based on the most recent
information available for the subject. Data collected in the 1992
NHEFS were used to update these two variables for subjects
successfully traced in that survey. If the subject was identified as
deceased in a previous survey, the information from that survey was
retained. If the subject was lost to followup in the 1992 NHEFS, the
vital status from the previous wave where the vital status was last
confirmed alive was retained. The Date Last Known Alive was coded
to the date of the NHANES I examination for subjects whose vital
status was unknown in the 1982-84, 1986, 1987, and 1992 Surveys.
The source flag stored in position 24 indicates the survey from which
the data in positions 17-23 were obtained. For more information on
the subject's vital status and date last known alive from the 1982-84,
1986, 1987, and 1992 NHEFS, please see positions 102-108, 128-134,
159-165, and 189-196 respectively.
(18-23) Date Last Known Alive
18-19 Month
01-12 = Month when last known alive
98 = Don't know
99 = Not ascertained
20-21 Day
01-31 = Day when last known alive
98 = Don't know
99 = Not ascertained
22-23 Year
71-93 = Year when last known alive (1971-1993)
NOTE: The Date Last Known Alive is either the date of last contact for
survivors and those lost to followup or it is the Date of Death for
decedents.
The Date Last Known Alive (positions 18-23) and the associated
Current Vital Status (position 17) are based on the most recent
information available for the subject. The following describes how
date last known alive is derived for each current vital status code.
1 = If the subject or a knowledgeable proxy respondent
answering on behalf of an incapacitated subject, completed
or partially completed either a telephone or mail
questionnaire, or if the subject or proxy answered
verification questions either during the tracing or interview
period without completing an interview, then the current
date last known alive would be equal to the date the
interview or the date the verification questions were
completed.
3 = The current vital status will be deceased if the subject has
been identified as deceased from a previous survey or if the
subject is coded as deceased for the 1992 NHEFS. For all
subjects for whom the current vital status is deceased the
current date last known alive will be equal to the date of
death.
4 = If the subject was considered not located for the 1992
NHEFS then the current date last known alive is equal to
the date last known alive in the 1987 NHEFS. If the
subject was not located at either the 1992 or 1987 NHEFS
the current date last known alive is equal to the 1986
NHEFS. If the subject was not located at the 1992, 1987,
and 1986 NHEFS then the current date last known alive is
equal to the 1982-1984 NHEFS. If the subject was not
located at any followup the current date last known alive is
the date of the NHANES I Examination.
5 = If the subject is traced alive with direct subject contact but
vital status verification questions could not be completed
then the current date last known alive is equal to the date of
the last direct subject contact.
6 = If the subject is traced alive without direct subject contact
but the subject's vital status is verified, then the current date
last known alive is equal to the date of vital status
verification.
24 Source of Current Vital Status and Date Last Known Alive
1 = 1982-84 NHEFS
2 = 1986 NHEFS
3 = 1987 NHEFS

29-32 Year
1895-1950 = Year of birth
NOTE: The NHEFS Date of Birth is the subject's birth date reported at
NHANES I. It was corrected for subjects for whom updated birthdate
information was obtained in the 1982-84 NHEFS. The NHEFS Date
of Birth was not updated using data collected from the 1986, 1987 or
1992 NHEFS.
(33-38) Date of NHANES I Examination
33-34 Month
01-12 = Month of NHANES I examination

NOTE: This is the original strata variable that was released with the NHANES
I Public Use data tapes. For more detailed information concerning this
variable, see Vital and Health Statistics, Series 1, Numbers 10a, 10b,
and 14 and the detailed notes for the NHANES I Public Use data tape
documentation. Two new strata variables have been created for use
with NHANES I public use data to facilitate variance estimation.
These revised strata variables are located in tape positions 256-257 and
259-260 on the 1992 NHEFS Vital and Tracing Status Public Use Data
Tape.
43-45 Pseudo Primary Sampling Unit (PSU)
1-235 = Pseudo primary sampling unit
NOTE: This is the original pseudo primary sampling unit variable that was
released with the NHANES I Public Use data tapes. For more detailed
information concerning this variable, see Vital and Health Statistics,
Series 1, Numbers 10a, 10b, and 14 and the detailed notes for the
NHANES I Public Use Data tape documentation. Two new pseudo
primary sampling unit variables have been created for use with
NHANES I public use data to facilitate variance estimation. These
revised pseudo primary sampling unit variables are located in tape
positions 258 and 261 on the 1992 NHEFS Vital and Tracing Status
Public Use Data Tape.

NOTE: This variable is the revised race variable that was created in the 1982-
84 NHEFS to resolve discrepancies between the NHANES I
interviewer-observed race and the 1982-84 NHEFS respondent-
reported race. Revised race codes were based on a case-by-case
adjudication of the following data: NHANES I race and ethnicity;
1982-84 NHEFS race and ethnicity; and, in the case of deceased
subjects, race on the death certificate that was obtained in the 1982-84
NHEFS. Information collected on race from the 1986, 1987 or 1992
NHEFS was not used to update this variable.
48 Marital Status When Last Interviewed
1 = Married
2 = Widowed
3 = Divorced
4 = Separated
5 = Never married
8 = Unknown
NOTE: This variable reflects the marital status reported in the most recent
interview for the subject.
49 Language Other Than English Spoken in Household at
NHANES I Examination
0 = German
1 = Italian
2 = French
3 = Polish
4 = Russian
5 = Spanish
6 = Chinese
7 = Other language
8 = Don't know
9 = Inapplicable, only English spoken
NOTE: This variable is extracted from NHANES I Baseline Public Use data
tape position 119.
50-51 Blank
INFORMATION PERTAINING TO DEATH: KNOWN DEATHS
52 Death Certificate Receipt Flag
1 = Death certificate has been obtained for deceased subject and death
certificate information is coded on the 1992 NHEFS Mortality Data
tape
2 = Death certificate was not obtained for deceased subject
Blank = Inapplicable, subject was not known to be deceased
NOTE: This variable identifies ALL known decedents for whom a death
certificate was obtained (N=4,482). Thus, it reflects the receipt of a
death certificate for 1,968 subjects identified as deceased at the 1982-
84 NHEFS (though thirty-three (33) of these certificates were received
after the end of the 1982-84 NHEFS survey period), 616 subjects
identified as deceased at the 1986 NHEFS, 539 subjects identified as
deceased at the 1987 NHEFS (though fifteen (15) of these certificates
were received after the end of the 1987 NHEFS survey period), and
1,374 subjects identified as deceased at the 1992 NHEFS.
53-56 Underlying Cause of Death
0000 = Subject deceased but does not have a death certificate on the 1992
NHEFS Mortality Data tape
0010-9999 = ICD-9 code (not inclusive)
Blank = Inapplicable, subject not known to be deceased
NOTE: Underlying cause of death is coded by rules of the International
Classification of Diseases, Ninth Revision (ICD-9). See the Manual
of the International Statistical Classification of Diseases, Injuries, and
Causes of Death: Based on the Recommendations of the Ninth
Revision Conference, 1975, and Adopted by the Twenty-Ninth World
Health Assembly, Volume 1, 1977 (World Health Organization) for a
complete list of codes. For injuries and poisonings, the external cause
is coded (E800-E999) rather than the Nature of Injury (800-999).
These positions do not include the letter E for the external cause of
injury. For those causes that do not require a 4th digit, position 56 is
blank.
57-60 Blank
(61-66) Date of Death
NOTE: If a death certificate was obtained for the subject, the subject's date of
death is extracted from the 1992 NHEFS Mortality data file. If a death
certificate was not obtained for the subject but a facility abstract was
returned which indicated that the subject was discharged dead from the
facility, the Date of Death is coded to the date of discharge entered on
the abstract form. If neither a death certificate nor a facility abstract
(indicating the subject was discharged dead) was available, the Date of
Death is coded from the information provided in the proxy interview.
This variable has been updated with the date of death stated on the
death certificate for one 1982-84 NHEFS decedent whose death
certificate was received after the end of the 1982-84 survey period.
The subject's year of death on the death certificate differed from the
year reported from the 1982-84 NHEFS proxy interview.

65-66 Year
71-93 = Year of death (1971-1993)
Blank = Inapplicable, subject not known to be deceased
67-68 Age at Death
30-96 = Age at death (in years)
Blank = Inapplicable, subject not known to be deceased
NOTE: Age at Death is calculated from the NHEFS Date of Birth and the Date
of Death and is not extracted from the 1992 NHEFS Mortality Data
tape. For subjects with missing date of death information, the
following convention was used to impute Age at Death: if the Month
of Death is missing, then month of death=7; if the Day of Death is
missing, then day of death=15; and, if both Month and Day of Death
are missing, then month of death=7 and day of death=1.
This variable reflects the change in the year of death for one 1982-84
NHEFS decedent whose death certificate was received after the end of
the 1982-84 survey period. The year of death on the death certificate
differed from the year reported from the 1982-84 NHEFS proxy
interview.

98 = Don't know
99 = Not ascertained
107-108 Year
71-84 = Year when last known alive (1971-1984)
NOTE: Date Last Known Alive is defined according to vital status (position
102) at the 1982-84 NHEFS. The determination for this variable
follows for each vital status code.
Vital Status Codes
(1) If the subject was alive and an interview was obtained interviewed at
the 1982-84 NHEFS, the Date Last Known Alive is the 1982-84
NHEFS interview date.
or
If the subject was alive and the result of an attempted 1982-84 NHEFS
interview was a nonresponse, the Date Last Known Alive is the date
of the attempted followup interview or the date when last contacted at
the 1982-84 NHEFS.
(3) If the subject is deceased, the Date Last Known Alive is the date of
death provided from either the death certificate or the proxy interview
obtained in the 1982-84 NHEFS. The Date Last Known Alive has
been updated with the date of death stated on the death certificate for
one 1982-84 NHEFS decedent whose death certificate was received
after the 1982-84 survey period. The year of death on the death
certificate differed from the date reported from the 1982-84 NHEFS
proxy interview.
(4) If the subject was never located at the 1982-84 NHEFS or the only
death information available was from an unconfirmed source, the Date
Last Known Alive is the date of the NHANES I exam.
(5) If the subject was found during tracing but was subsequently lost prior
to the 1982-84 NHEFS interview period, the Date Last Known Alive
is the date of last contact or the mid-point date of tracing for each
specific regional area. These are:
Pretest: December 1, 1980 to April 1, 1982
Mid-point = August 1, 1981
Northeast: February 1, 1981 to August 1, 1982
Mid-point = November 1, 1981
South: April 1, 1981 to January 1, 1983
Mid-point = February 1, 1982
Midwest: June 1, 1981 to April 1, 1983
Mid-point = May 1, 1982
West: September 1, 1981 to July 1, 1983
Mid-point = August 1, 1982
NHANES I stand was used to group cases into regional tracing areas.

a 1982-84 NHEFS interview was administered but is missing one or
more sections of information. Respondents who broke off the
interview after completing section D (Health/Diseases and Operations)
are also coded as partial complete.
An interview disposition of refusal/breakoff (code 03) indicates a
1982-84 NHEFS interview was not administered or the respondent
broke off the interview before completing section D. Interviews
ending before completion of section D were not coded.
Interview information is available on the 1982-84 NHEFS Interview

NOTE: The Date Last Known Alive is defined according to vital status
(position 128) at the 1986 NHEFS. The determination for Date Last
Known Alive for each vital status code follows.
Vital Status Codes
(1) If the subject was alive and an interview was obtained at the 1986
NHEFS, the Date Last Known Alive is the date when the 1986 NHEFS
interview was conducted or the date when the subject's mail questionnaire
was received.
or
If the subject was alive and the result of an attempted 1986 NHEFS
interview was a nonresponse, the Date Last Known Alive is the date of the
attempted interview or the date of last contact in the 1986 NHEFS.
(3) If the subject was identified as deceased in the 1986 NHEFS, the Date
Last Known Alive is the date of death provided from a death certificate,
a facility abstract, or a proxy interview (in that order of priority) obtained
in the 1986 NHEFS.
(4) If the subject, at the 1982-84 NHEFS, was either never located or was
identified as deceased but that information was obtained from an
unconfirmed source and the subject, at the 1986 NHEFS, was either never
located or was identified as deceased in the 1986 NHEFS but that
information was obtained from an unconfirmed source, Date Last Known
Alive is the date of the NHANES I examination.
or
If the subject was successfully traced at the 1982-84 NHEFS but then, in
the 1986 NHEFS, either was never located or was identified as deceased
at the 1986 NHEFS and that information was obtained from an
unconfirmed source, Date Last Known Alive is the date last known alive
at the 1982-84 NHEFS.
(5) If the subject was located during the 1986 NHEFS tracing period but was
subsequently lost prior to the interview period, the Date Last Known
Alive is the date the subject was located during tracing at the 1986
NHEFS.
(7) If the subject was traced alive at the 1986 NHEFS but no direct contact
was made with the subject, the Date Last Known Alive is the date tracing
was conducted in the 1986 NHEFS.
(8) If the subject's age was less than 55 at the NHANES I exam, the Date Last
Known Alive is coded as Not Eligible (989898).
(9) If the subject was identified as deceased in the 1982-84 NHEFS, the Date
Last Known Alive is coded as Inapplicable (999999).
(135-140) Date of Interview: 1986 NHEFS

the respondent broke off the interview in the 1986 NHEFS at some
point after completing section A (Background Information) of the
questionnaire and did not complete the remainder of the interview.
Furthermore, it indicates that the respondent provided information on
at least one facility stay for the subject in section B (Medical
Conditions) of the questionnaire or completed a substantive part of
section B, if the respondent did not fully complete that section.
An interview disposition of refusal/breakoff (code 03) indicates that a
1986 NHEFS interview was not administered or the respondent broke
off the interview before completing section A or the respondent broke
off the interview early into section B without providing information on
any facility stays for the subject. In the latter two cases, the interviews
were not coded.

health care facility stay information collected at the 1987 NHEFS for
each of the subjects who were at least 25 years old at the NHANES I
examination who were not known to be deceased in the 1982-84 or
1986 NHEFS. More detailed information with respect to these
variables may be obtained from the 1987 NHEFS Interview, Health
Care Facility Stay, and Mortality Data Public Use data tape
documentation.

164-165 Year
71-88 = Year when last known alive (1971-1988)
99 = Inapplicable, 1982-84 or 1986 NHEFS decedent
NOTE: The Date Last Known Alive is defined according to vital status
(position 159) at the 1987 NHEFS. The determination for Date Last
Known Alive for each vital status code follows.
Vital Status Codes
(1) If the subject was alive and an interview was obtained at the 1987
NHEFS, the Date Last Known Alive is the date when the 1987 NHEFS
interview was conducted or the date when the subject's mail questionnaire
was received.
or
If the subject was alive and the result of an attempted 1987 NHEFS
interview was a nonresponse, the Date Last Known Alive is the date of the
attempted interview or the date of last contact at the 1987 NHEFS. The
Date Last Known Alive was corrected for five cases where the month and
day last known alive were found to be incorrect. Year last known alive
was not affected by this recode.
(3) If the subject was identified as deceased in the 1987 NHEFS, the Date
Last Known Alive is the date of death provided from a death certificate,
a facility abstract, or a proxy interview (in that order of priority) obtained
in the 1987 NHEFS.
(4) If the subject had an unknown vital status in the 1982-84, 1986 and 1987
NHEFS surveys because the subject was never successfully traced or was
identified as deceased but that information was obtained from an
unconfirmed source, the Date Last Known Alive is the date of the
NHANES I examination.
or
If the subject was successfully traced at the 1982-84 or 1986 NHEFS but
then, in the 1987 NHEFS, either was never successfully traced or was
identified as deceased and this information was obtained from an
unconfirmed source, Date Last Known Alive is the date last known alive
at the 1982-84 or 1986 NHEFS, depending on when the subject was last
located.
(5) If the subject was located during the 1987 NHEFS tracing period but was
subsequently lost prior to the interview period, the Date Last Known
Alive is the date the subject was located during tracing at the 1987
NHEFS.
(7) If the subject was traced alive at the 1987 NHEFS but no direct contact
was made with the subject, the Date Last Known Alive is the date tracing
was conducted in the 1987 NHEFS. The Date Last Known Alive was
corrected for one case where the month and day last known alive were
found to be incorrect. Year last known alive was not affected by this
recode.
(9) If the subject was identified as deceased in the 1982-84 or 1986 NHEFS,
the Date Last Known Alive is coded as Inapplicable (999999).
(166-171) Date of Interview: 1987 NHEFS
NOTE: The Date of Interview is the date when the subject or proxy interview
was conducted or the date when the mail questionnaire was received
in the 1987 NHEFS. The 1987 NHEFS Date of Interview was
corrected for 2 cases where the month and day of interview were later
found to be incorrect. Year of 1987 interview was not affected by this
recode.

71-93 = Year when last known alive (1971-1993)
99 = Inapplicable, 1982-84, 1986 or 1987 NHEFS decedent
NOTE: The Date Last Known Alive is defined according to vital status
(position 189-190) at the 1992 NHEFS. The determination for Date
Last Known Alive for each vital status code follows.
Vital Status Codes
(01) If the subject was alive and an interview was obtained at the 1992
NHEFS, the Date Last Known Alive is the date when the 1992 NHEFS
interview was conducted or the date when the subject's mail questionnaire
was received.
or
If the subject was alive and the result of an attempted 1992 NHEFS
interview was a nonresponse, the Date Last Known Alive is the date of the
attempted interview or the date of last contact at the 1992 NHEFS.
(03) If the subject was identified as deceased in the 1992 NHEFS, the Date
Last Known Alive is the date of death provided from a death certificate,
a facility abstract, or a proxy interview (in that order of priority) obtained
in the 1992 NHEFS.
(04) If the subject had an unknown vital status in the 1982-84, 1986, 1987, and
1992 NHEFS surveys because the subject was never successfully traced
or the subject's vital status could not be confirmed or the subject was
identified as deceased but that information was obtained from an
unconfirmed source, the Date Last Known Alive is the date of the
NHANES I examination.
or
If the subject was successfully traced at the 1982-84, 1986 or 1987
NHEFS but then, in the 1992 NHEFS, either was not successfully traced
or the subject's vital status could not be confirmed or the subject was
identified as deceased and this information was obtained from an
unconfirmed source, Date Last Known Alive is the date last known alive
at the 1982-84, 1986 or 1987 NHEFS, depending on when the subject was
last located.
(05) If the subject's vital status was confirmed either during tracing or during
an attempt to complete an interview, but no interview was obtained, the
Date Last Known Alive is the date of the vital status confirmation.
(06) If the subject was traced alive at the 1992 NHEFS but no direct contact
was made with the subject, the Date Last Known Alive is the date tracing
was conducted in the 1992 NHEFS.
(99) If the subject was identified as deceased in the 1982-84, 1986 or 1987
NHEFS, the Date Last Known Alive is coded as Inapplicable, (999999).
(197-202) Date of Interview: 1992 NHEFS
NOTE: The Date of Interview is the date when the subject or proxy interview
was conducted or the date when the mail questionnaire was received
in the 1992 NHEFS.

262 Poverty Area Indicator at NHANES I Examination
1 = Poverty area
2 = Non-poverty area
3 = Augmentation sample
NOTE: This variable indicates whether the subject's household was located in
a poverty area during the NHANES I examination period. Poverty
areas were defined using census enumeration districts (ED) or segment
areas (a cluster of households). The census designation of "current
poverty area" is described in detail in Appendix !, Vital and Health
Statistics, Series 1, Number 10a. Non-poverty areas were defined as
all other ED's or segment areas not previously designated as current
poverty areas by the Census Bureau.
The sampling design of the NHANES I included oversampling of
persons living in poverty areas during the first stage of the data
collection period (1971-1973). The NHANES I study was then
extended through 1974-1975 to include an augmentation sample for
which there was no oversampling in poverty areas and therefore, no
information regarding household location is available for this group.
