Kirkuchi's lymphadenitis in HIV infection: a case report. Stefang Pileri P. Baglioni.

OBJECTIVE: The authors report on a case of "histiocytic necrotizing lymphadenitis/Kikuchi's lymphadenitis" (HNL) which occurred in a 17 yr-old female with previous history of drug-addiction. CASE REPORT: The patient seroconverted in 1986 and developed ARC (CDC IVa-WR5a) in July 1988. At that time, she showed generalized lymphadenopathy, lymphopenia (321/mmc) and CD4/CD8 ratio = 0.08. Flow cytometry and double fluorescence displayed a significant expansion of the HLA-D/DR+ subset, which was not due to an absolute increase in activated (CD3+) T-lymphocytes. A Jamshidi needle biopsy revealed moderate myelodisplasia with plasmocytosis. In October 1988, the patient underwent a lymph node biopsy. At histological examination, the normal structure was effaced by multiple foci of infiltration by histiocytes, plasmocytoid T-cells, small lymphocytes, and immunoblasts, with nuclear debris and varying degrees of necrosis. Immunophenotyping showed positivity of: i) histiocytes for lysozyme and CD11b, CD11c, CD14, and Ber-Mac3 molecules; ii) plasmocytoid T-cells for CD45 and CD10; iii) small lymphocytes for CD2, CD3, and CD5 (CD4/CD8 = 1/5). According to morphological and immunohistochemical findings, a diagnosis of Kirkuchi's lymphadenitis was made. CONCLUSION: To our knowledge, this represents the first reported example of HNL in an HIV-infected patient. It underlines the opportunity of including HIV among the possible etiologic agents of Kirkuchi's lymphadenitis.
