OBJECTIVE: To investigate the expression of T cell receptor (TCR) isotype and the mucosal T cell-associated activation marker HML-1 in the intestinal mucosal immune system of HIV-infected patients and controls. METHODS: 12 male HIV-infected patients (age 26-52, median 32 years) and 15 controls (8f, 7m; age 18-86, median 61.5 years) were studied. Cryostat sections of biopsies or resected specimens were stained immunohistologically (APAAP) with monoclonal antibodies T3, HML-1, TCRdelta1, and betaF1. The number of stained IEL per 100 epithelial cells, and of stained LPL per 100 mononuclear lamina propria cells was determined microscopically. Results were compared by the two-tailed Mann-Whitney U test (*=p less than 0.05). RESULTS: The number of HML-1+ intraepithelial lymphocytes and the ratio HML-1+/CD3+ was decreased in HIV-infected patients compared with controls, while no differences were found in alpha beta, gamma delta, or CD3+ T cells. In the lamina propria, a reduction of alpha beta, CD3+, and HML-1+ T cells, and of the ratio HML-1+/CD3+ was found in HIV infection; the number and the proportion of gamma delta T cells were not altered. TABULAR DATA, SEE ABSTRACT VOLUME. CONCLUSIONS: Loss of T cells in HIV infection is confined to alpha beta T cells in the lamina propria which might reflect the absence of CD4 on gamma delta T cells. However, reduced HML-1 expression indicates functional impairment of both lamina propria and intraepithelial lymphocytes, although the latter express rarely CD4.
