Virologic analysis of long term HIV seropositive asymptomatic individuals.

Introduction. Intensive virologic analysis was performed on asymptomatic-long term survivors compared to recent seroconverters. Methods. Groups studied; Healthy long term survivors (HLP, n=35); seropositive greater than 10 years, asymptomatic, CD4 greater than 500/mm3; recent seroconverters (RSC, n=22); seroconversion since 1987, asymptomatic, CD4 greater than 500/mm3. Quantitative plasma viremia, MT2 assay, qualitative and quantitative PBMC cultures were performed. Results. Plasma viremia was significantly lower in the HLP's compared to RSC's. A negative correlation was observed between plasma viremia and increasing CD4 counts in HLP's. HIV was isolated in all but one HLP and in all RSC's. The presence of syncytium forming HIV was detected with lower frequency in the HLP's. Four HLP's had a low plasma viremia (less than 200 copies/ml, greater than or equal to 2 sampling) while others had a high plasma viremia (greater than 50,000 copies/ml, greater than or equal to 2 sampling) with an unchanged CD4 count. One HLP showed a decreasing trend in plasma viremia (32,000 to 4000 over 16 months) while in another, increased plasma viremia and Sl phenotype correlated with progression. Conclusion. HLP's were clearly defined by lower levels of plasma viremia compared to RSC's. Within the HLP group, this virologic analysis demonstrates a heterogeneous group including low plasma viremia and difficult isolation of HIV, high level of plasma viremia and easy virus isolation, increasing level of plasma viremia and finally one HLP with a decrease in plasma viremia.
