OBJECTIVES: Prevalence data of Helicobacter pylori (H.p.) and gastric or duodenal ulcer disease (GU/DU) in HIV+ pat. are very rare and at variance. METHODS: We conducted a prospective age-matched study on point-prevalence of epigastric pain, H.p. positivity and GU/DU between 1/1989 and 5/1992 involving 25542 anti HIV- controls and 1043 HIV+ pat. presenting in our out-patients clinics. RESULTS: 841 (3.3%) of the HIV- controls complained of epigastric pain. The frequency in HIV infected was about 10 times higher (385/10435 = 36.9%). 417 controls (mean age 57.4y) were endoscoped. 173 patients were H.p. positive in histology and/or in HLO-test (41.5%). 90 had duodenal (21.6%) and 74 (17.7%) gastric ulcer diagnosed. The prevalence in HIV infected patients was 0% (0/219) and 0% (0/219) for duodenal and gastric ulcer, respectively. Age-matched prevalence data (%) were as follows: TABULAR DATA, SEE ABSTRACT VOLUME. Especially patients on Zidovudine and/or on recurrent antibiotics had a low H.p. prevalence. CONCLUSIONS: Although epigastric pain was 10 times more frequent this study confirmed a much lower prevalence of gastric H.p. infection in HIV+ compared to HIV- patients. Prevalence of gastric or duodenal ulcer disease was zero in HIV positive patients. The zero prevalence of peptic ulcer in HIV+ patients might therefore be attributed to the compromised immunofunction and/or antibiotic treatment of this group.
