STD/Aids Coordination of Sao Paulo City Health Department, Sao Paulo, Brazil

Issues: To describe the evolution of the distribution of antiretroviral medicines and its tendency to decrease the mortality rates in Aids cases in the City. Description: Sao Paulo is the biggest city in Brazil, with a population of, 11 millions people. According to theSTD/Aids National Coordination 257.780 cases of Aids were notified in Brazil, from the begining of the epidemic until December 2002. The city of Sao Paulo is responsible for the majority of cases: 55.772 (1980-2003). After the inauguration of the current local government, the STD/Aids Coordination of Sao Paulo City Health Departament did a diagnosis for the actual situation. It showed that there was no institutional policy for STD/Aids, and so different actions started to be taken. The Coordination was divided in different sectors, each one of them having its action plan, that work together as a whole. The reorganizing of the databank of STD/Aids Epidemical Surveillance and the establishment by Pharmaceutical Assistance of new databank for patients using antiretroviral therapy were most outstanding. Analyzing the data: The number of patients receiving antiretroviral medicines in the city was increased, as follows:2001 - monthly average - 5.671 patients,2002 - monthly average - 6.245 patients,2003 - monthly average - 7.093 patients,As a result the decrease in the number of death cases:2001 - 898,2002 - 676 and,2003 - 170. Recommendations: The structuring of a free of charge antiretroviral medicines and an efficient system of Aids cases notification showed an amazing reduction of hospitalization rates and mortality of HIV positive patients. These measures can be internationally adopted against Aids epidemic.
