Subject: Defects Potentially Reportable Under 10CFR21
Gentlemen:
We are writing to inform you about a possible material defect in certain
stainless steel Swagelok tube fittings made from forgings.
A list of the part numbers and the U.S. nuclear facilities to which these
parts have been sold is enclosed. All of these facilities have already
been notified by us about this possible problem. The potential defect is
limited to the heat codes listed with the part numbers.
The flaw in the forging is from an erogenous source of contamination on
the stainless steel. This causes a weakness in the material to form
during the forging process. None of the flaws have ran completely
through the part, but the flaw may or may not be visually identifiable.
Less than 0.1% of the parts contain this defect.
We enclose certified material test reports for each of the heat codes
from the material organizations. We have already modified our handling
and processing of stainless steel raw material to avoid any future
contamination.
Sales drawings for each of the referenced part numbers are included with
this letter.
