BACKGROUND: While complications of A.P in children are being well established, only very few studies evaluate long-term complications in women with A.P. The aim of this study was to evaluate renal complications after more than 10 years of follow-up of women with A.P. METHODS: Women who were hospitalized over 10 years ago at Ha'Emek Medical Center, and diagnosed with A.P, were invited to participate. The protocol included demographic and clinical characteristics, underlying diseases, and all details related to the urinary tract, such as infections in childhood, pregnancy and recurrency. After clinical evaluation, renal function test, renal ultrasound and DMSA renal scan were conducted. RESULTS: Sixty three women participated in the study. Renal scars by DMSA scan were present in 29 (46%) women. The mean age at hospitalization was 26.6 + 7.3 years for the women with scars (Group A), and 35.0 + 10.9 years for those without scars (Group B), p=0.001. Mean follow-up age was 17.3 + 6.2 and 16.4 + 3.1 years, respectively. UTI in childhood was reported in 5 women in group B, and none in Group A. In addition, 19 (65.5%) women with scars had UTI during pregnancy, in comparison to 10 (34.5%) women with no scars (p=0.004). The mean GFR was lower in the women in Group A: 105.3 + 29.0 vs. 88.0 + 24.0 ml/min (p=0.01). However, no difference in the occurrence of hypertension or recurrent UTI was noted. A relative reduction in renal function by the DMSA scan was observed in 25 women in Group A (86.2%) and only 11 (32.4%) in Group B (p<0.001). A multivariate model showed that serum albumin less than 3.2 gr% (O.R 12.5) and A.P in pregnancy (O.R 3.9) were significantly associated with the presence of renal scars. CONCLUSIONS: In contrast to previous concepts renal scars are very frequent in women with A.P especially in those pregnant and with hypoalbuminemia. In spite of a moderate reduction in GFR no serious clinical complications after more than 15 years follow-up were observed.
