BACKGROUND and METHODS: Since 1994 Jan. 1st, a new blood use policy has been established in the hospitals of Bassano del Grappa, Veneto Region, Italy, in order to modify inappropriate physicians' practice. The Hospital Transfusion Committee developed clinical practice guidelines for blood therapy based on the critical appraisal of the literature and the local needs. The strategy represented a combination of methods in which the specialist of the Blood Transfusion Service was given the special role of discussing with physicians about each inappropriate blood components ordering before filling it, ensuring the clinical autonomy in any case. RESULTS: The strategy resulted in lowering the number of transfused patients (1993: 1696; 1994: 1498; = -12%) and the utilization of blood units (1993: 4330; 1994: 3686; = -15%), transfusion labor tests (1993: 26,655; 1994: 20,866; = -22%) and albumin units (1993: 4701; 1994: 3128 = .33%). CONCLUSIONS: Further research is needed to put this method as the potential of rationing clinical practice by creating barriers to inappropriate use of limited health care resources.
