Developing a national prevention programme for mother to child transmission hiv prevention.

ISSUES: hiv/aids epidemic has harly devastated the honduran society. During the period 1987- 2001 under 5 years old have been reported with hiv infection or aids, 95% due to perinatal transmission. In the same perod, 12,490 caser of aids were reported 40% female be coming the frist death cause ( 18%) in reproductive age women . DESCRIPTION: supported by unicef and unaids the ministry of health started by april/99, a pilot project to prevent the transmission of hiv from mother to to child (pmtct) in the main cities, tegucigalpa and san pedro sula, further amplified to additional 11 cities in the next two year, in order to establish a national pmtct program. Zdv was the drug delivery and postnatal selected to use in the protocol. RESULTS: an integral prenatal protocol was elaborated for hiv positive pregnant women . traiming of 156 counselor (phisicians, nurses, psychologists and social workers) from 19 health centers and 6 hospitals was achieved. As of december /01 9,583 new prenatal attention were reported, with 3,527 pre-test counselings, 2,479 hiv tests done finding, 28 seropositive women with 16 children found repetitive elisa reactive all of which were fed with formula according to mother decision; pcr test done showed 6 seronegative children. LESSONS LEARNED: beginning a pmtct pilot project is a good plataform to launch a national program an integral care focus and straightforward coordination with all involved institution and organization campaigns to increase awareness and promotion of the hiv test and the utilization of the health services, must be a remarkable advocacy at central and local level, logistics, and monitory and evaluation, are also relevant issues to be included. Recomendation: pmtct program is being the beginning of a wide antiretroviral therapy program wondered to be active by mid of 2002. Seropositive women and their ptoducts are a priority to receive hiv drugs.
