Intervention with female sex workers & clients in Madras.

OBJECTIVE: To reduce risk of HIV transmission among 500 women in prostitution and their clients (800 daily) in Kodambakkam, Vadapalani, KK Nagar sections of Madras. METHODS: Training of Peer Leaders from among Sex Workers as change agents; use of third parties of male and female brokers (aunties) as agents for change in spreading awareness of STD/HIV/AIDS, promoting safer sex and sales of condoms. To reach out to women and men in commercial sex areas to increase awareness of STD/HIV/AIDS, increase health seeking behavior and use of condoms. To provide both social and health services including STD care and counseling. The project was a pilot project to test strategies. Duration of the project was 6 months (extended to one year). It was a first attempt to target hidden and underground commercial sex circuits in an urban area typical for South India, with AIDS prevention. Ethnographic research was carried out to design intervention plan. Baseline surveys were conducted before intervention and after 6 months. Community Action Network was implementing party. WHO/GPA provided technical assistance and funds. Project got full backing AIDS Cell, Government of Tamil Nadu and the National AIDS Control Organization. RESULTS: An increase in sales of Fiesta condoms by 21% was achieved. Most of the sales were realized by affiliates from among the target groups. Availability of condoms in 11 out of 17 of the underground brothels in the area and at other places where men go with sex workers such as lodges, places of entertainment etc. Provision of STD services in cooperation with the Madras Municipal Corporation integrated with general health services in public health dispensary in the target area. Significant increase in awareness levels of STD/HIV/AIDS. CONCLUSIONS: Enabled design of strategies for reduction of HIV transmission in the covert and underground circuit of prostitution highly stigmatised and repressed which could be adapted for the entire South India where situations are similar.
