Provision of pre- and post test counseling to antenatal, postnatal HIV positive patients in a public hospital.

Issues: Unsafe blood, unsafe sex and vertical mode of HIV transmission are all seen among patients of a maternity hospital. Despite absence of specifically trained counselors, educating and counseling patients to prevent vertical transmission was important. Provision of free obstetric, counseling and paediatric services to illiterate semieducated, low economic status patients on a zero budget safe motherhood program. Description: The largest maternity hospital in Asia employed techniques of group orientation to all registered patients on HIV/AIDS and vertical prevention, followed by free serum testing with verbal consent (50 patients/day). HIVpositive identified couples offered post-test counseling. Couples decided to undertake and self-finance modified 076 protocol and follow-up for 24 months to ascertain child's serostatus at 18 months age. Hospital social counselor undertook entire counseling for each couple for a total of 30 months. From 1994-1999, nearly 1000 couples benefited from the program. Ongoing adjustments, creating family support system, alternate-healing methods, legal help for sociolegal problems. Conclusion: Existing hospital resources put to optimal use. Culture specific, acceptable education strategies to reach large numbers designed. Perception of counseling by patients as excellent, timely, indispensable, valuable tool for prevention. Effective liason between obstetrician, counselor and paediatrician. Education and counseling are extremely challenging but indispensable for uneducated, illiterate HIV positive persons and pregnant mothers.
