OBJECTIVE: 118 autochthonous patients with AIDS underwent autopsy at the Nice University Medical Center, from May 1987 to December 1990. The incidence of the infectious and neoplastic diseases was assessed by post mortem tissues' study. METHODS: Complete and consecutive autopsies were performed on 118 patients fulfilling the Center for Disease Control criteria for AIDS. There were 74 caucasian males and 44 caucasian females. The average age was 31 years (range 21 to 74 years) and 75% were between the age of 25 and 35 years. Eighty one patients were intravenous drug abusers, 22 were homosexuals and 7 had received multiple transfusions; 8 had no identifiable risk factors. After the clinical symptoms, the mean duration of the disease was 11 +/- 4 months. RESULTS: Microorganisms. Pneumocystis carinii (64 cases) Toxoplasma gondii (41) Cytomegalovirus (21) Cryptococcus neoformans (11) Mycobacterium avium intracellulare (9) Mycobacterium tuberculosis (5) Leishmania infuntum (5) Cryptosporidium species (4) Aspergillus fumigatus (4). Neoplastic diseases. Non Hodgkin's lymphoma (14): central nervous system lymphoma (9) and disseminated lymphoma (5); Hodgkin's disease (7) Kaposi's sarcoma (8): cutaneous (4) and disseminated (4); carcinoma (5): bronchioloalveolar carcinoma (2) cloacogenic carcinoma (2) and squamous carcinoma (1). CONCLUSION: this post mortem study has concerned a great majority of intravenous drug abusers (69%) Pneumocystis carinii and toxoplasmosis occured respectively in 54% and 35% of the cases. A neoplasm was observed in 28% of the cases. Some inusal tumoral as Hodgkin's disease and bronchioloalveolar carcinoma were noted in this AIDS population.
