Many countries with limited resources have not been able to implement the PACTG 076 zidovudine regimen because of its complexity and cost. The objective of this study was to compare nevirapine, short-course zidovudine, and short-course zidovudine plus lamivu-dine regimens for safety, effectiveness, ease of administration, and cost-effectiveness in the prevention of MTCT of HIV-1 infection in resource-poor countries; members of the USP Drug Information Expert Committees reviewed the published clinical trials on nevi-rapine, short-course zidovudine, and short-course zidovudine plus lamivudine regimens presented to them in the form of evidence tables. Published clinical trials were analysed based on their evidence ratings. The evidence tables were based on evidence ratings. The main outcome of the study was safety, effectiveness, ease of administration and cost-effectiveness; The short-course zidovudine and short-course zidovudine plus lamivudine regimens were inferior to that of the PACTG 076 regimen. The administration of a single dose of nevi-rapine to the mother intrapartum and to the infant postpartum effectively reduces MTCT of HIV-1 and is less costly and easy to administer than the other antiretroviral regimens; however, mutations conferring resistance to nevirapine could be observed after a single dose even with a low level of viral replication. Based on these results, members of the USP Drug Information Expert Committees recommended that in the developed world the use of nevirapine should be restricted to previously untreated women with HIV-1 infection who present in labour; however, in resource-poor countries nevirapine may be considered as one of several strategies in the prevention of MTCT of HIV-1 infection.
