Marriott Ballroom Salon 2 (Lobby Level) This session will provide an overview of the HCAP program including resources and reporting requirements. Newly-funded grantees and new Project Directors also will hear from experienced grantees on how to successfully collect data and complete the 6-Month Report, getting started with the local evaluation, and other lessons learned. Additional time will be allotted for questions and answers and for new grantees to meet with their project officer. This session is open to all new grantees and new staff of continuing projects.

Marriott Ballroom Balcony A (Mezzanine Level) This session will guide participants through the key elements of developing a comprehensive business plan for an HCAP program and is recommended for CEOs, senior management staff, and Project Directors. Participants will learn the fundamental components of business plans, critical success factors in business planning, successful monitoring and evaluating techniques, and essential steps to initiate the business planning process. The case study method will be used to provide participants with an effective experiential learning process. Participants are encouraged to bring copies of their business plans for discussion purposes and consultation with session instructors, time permitting.

1:30 p.m.-5 p.m. Return on Community Investment (ROCI)
Marriott Ballroom Balcony B (Mezzanine Level) ROCI is a method that managers of community coalitions are using to demonstrate the outcomes and cost savings of community health initiatives so that community partners continue to put community resources together to add value in achieving results. It's "deal making" for sustainability. It takes a new kind of strategic manager to facilitate agreement on the value to be delivered in exchange for a specified resource investment. The future development challenge is to not only broker the deals to finance these critical activities, but to build them to the scale where a sustainable, integrated health care delivery system emerges at the local level. To be successful, this new breed of strategic manager will need to 1) produce outcomes others will value, 2) figure out the deals to make, and 3) create a certain level of community readiness. The defining and unifying discipline for exercising this new strategic management responsibility is called "Return on Community Investment" or ROCI. ROCI approaches the deal from the perspective of an investment to transform the whole health care safety-net in a community. The promise is that this new delivery system will perform in a significantly better way that produces results and returns that will make pooled investments attractive. This session will address the four elements of the method: enterprise, value, readiness, and game plan.

1:30 p.m.-5 p.m. Getting Started with Your Local Evaluation
Marriott Ballroom Balcony D (Mezzanine Level) This session features an evaluator who has worked with numerous HCAP grantees on their local evaluations. She will share her experiences and lessons learned in formulating local evaluations. Topics to be addressed include establishing evaluation contacts, identifying priorities, and types of evaluation approaches that fit with individual projects.

Marriott Ballroom Salon 2 (Lobby Level) Many HCAP programs implement some type of Management Information System. This session will highlight some of the models used by HCAP grantees such as Disease Management Systems, Practice Management Systems, and Eligibility Screening and Referral Systems. There will also be a focus on Electronic Health Records and the preparation necessary before implementation.
