Background: The first HIV infection has been described on Reunion Island in 1987. We have realized a prospective study of new HIV infections for epidemiological follow-up. Methods: Anonymous study based on laboratory results of Reunion residents with a first positive serology for HIV between September 1996 and December 1999. Results: 102 cases have been included. The rate of detected HIV infections has been steady with an average of 2.5 new positive serologies per month. 70% are men, 51% of patients are between 25 and 34 years old. The way of contamination is homosexual in 32% (45% of male patients), heterosexual in 52% and intravenous drug abuse in 9% of patients. A third of HIV-testing has been realized on patients' demand, 6.6% of testing was done during pregnancy, 16% of the tested patients had risk factors for HIV infection. In one third of cases, symptoms of HIV infection were present, a quarter of the patients had already developed Aids. Signs of primoinfection were present in 10% of the patients. Conclusion: During the study period, the rate of new HIV infections has been stable. However, increasing heterosexual contamination and a high proportion of patients diagnosed in stade of Aids show the need for emphasis on HIV prevention and earlier diagnosis.
