Reunion Island: Impact of HAART (Highly Active Anti-Retroviral Therapy) and epidemiological aspects of HIV cohort from 1995 to 1999.

Background: Reunion Island is an Overseas French Territory in the Indian Ocean (pop: 700.000) who benefits from the French Social Security system and of a full access to treatments registered in France. The first HIV case was registered in 1987. There are 2 types of patients: either locally contaminated or already infected immigrants. Method: Retrospective analysis of the 1995-1999 local database (``DMI2'') who gathers the results from the 2 AIDS hospital units of the Island (St Denis / St Pierre). Results: - Epidemiological data: Cohort raised from 287 to 445 patients (+55%) with a great stability of the sex-ratio (men 70%, women 30%), the age distribution (average 35 years, 50% between 30 and 40) and the transmission modes (heterosexual 45%, homosexual 35%, injecting drug users 15%). - Clinical status: Total number of death fell from 25 (rate: 8,7%) to 10 (2.2%), while AIDS new declarations has been reduced from 36 (12.5%) to 11 (2.5%). The total number of hospitalizations have also been reduced from 131 (45.6% of the cohort, average length 15.2 days) to 46 (10.3%, average length 17.8 days). - CD4 status: In 1999, 41% of the patients have more than 500 CD4/mm3 (14% in 1995), 43% between 200 and 499 (43% in 1995) and 16% less than 200 (43% in 1995). - HAART treated patients rate raised from 1% in 1995, to 36% in 1996, 60% in 1997, 82% in 1998 and 97% in 1999. Conclusion: During the 5 past years, Reunion Island experienced an increase of its cohort while HAART improved the average CD4 status plus the numbers and rates of death and hospitalization.
