1. ATSDR > Public Health Assessments & Consultations

1. Using the findings of this public health assessment and identified data gaps, ATSDR considers this site an indeterminate public health hazard. This conclusion is primarily based on the data gaps pertaining to the safety of the aggregate under conditions of chronic exposure, and its use in areas where dust generation and subsequent inhalation may occur.

2. A review of ambient air data does not indicate a present threat to human health via this pathway. Because of the lack of data on air during early operations at MSP, it is not possible to assess exposure to past reported emissions. The state now has in place adequate measures to monitor the air quality at the site. There are no provisions for off-site air monitoring.

3. Levels of mercury found in the 1986 fish sample may indicate potential risk to human health. Follow-up data were not available to allow comparison of the mercury levels during other periods. Environmental information for the site does not indicate that mercury is a significant contaminant.

4. The leachability of heavy metals from the aggregate at environmental pH should not pose a threat to public health. Exposure to metals by ingestion of the aggregate does not appear to result in acute toxicity. There are indications that metals in the aggregate are bioavailable. There is an information gap on body burden resulting from chronic ingestion of the aggregate. There is also a data gap on the fibrogenic potential of small particles released during crushing of the aggregate. Inhalation of the particles could be an important route of exposure for workers who place the product on roadways and other sites where crushing can occur. There is insufficient information to conclude that the aggregate is or is not a potential threat to human health if it is used in areas where crushing and dust generation occur. Available information on the aggregate at one residence has been evaluated; it suggests there may be a potential threat to public health at the residence. Aggregate at the Amelia Community Health Center is under a concrete slab and should not pose a threat to public health. Other sites where aggregate has been placed have not been individually evaluated. The mixed nature of the aggregate makes it difficult to address the public health implications of its general use. Until more is known about the bioavailability of the metals in, and the friability of, the aggregate, its use in areas where children might contact it should be discouraged.

5. Sampling has indicated on-site soil contamination and a contaminated groundwater plume under the site. Control measures have been instituted and monitoring of those areas should continue. Given the apparent migration path toward Bayou Boeuf and subsequent mitigation measures, the contamination is not expected to affect human health.

6. There does not appear to be a significant potential for contamination of peaches and pecans by aggregate in nearby yards. Leachability is not significant at environmental pH, and migration through the soil should not occur.

7. An excess number of neuroblastomas was reported in St. Mary parish from 1986 to 1987. Neuroblastoma has not been consistently linked to any single environmental agent. In the absence of a specific exposure to test, it is not possible to either link or rule out a link between the cases and any single facility.

8. St. Mary and surrounding parishes have higher than expected reported rates of congenital malformations. Those rates were elevated before the facility went on line. A sharp increase in the number of malformations in these parishes was reported in 1986. However, several other parishes in northern Louisiana experienced similar increases, so it is difficult to determine whether that information is site specific.

9. There are many parameters of health concern in this area, such as the excess rate of spina bifida. It is not possible to link those problems to the MSP facility.

1. Further study of the aggregate is needed to determine the body burden resulting from chronic exposure and the fibrogenic potential of small particles. Chronic, bioavailability studies are needed to determine the safety of the aggregate for use in areas where children might have chronic exposure to the aggregate. Fine scanning electron microscope studies would help characterize the type and shape of particles formed from crushing of the aggregate.

2. Monitoring of on-site soil and groundwater contamination should continue until the existing plume has been eliminated. Monitoring helps ensure that those sources of contamination are evaluated for possible changes that could affect human health.

3. To allay concerns about the peaches and pecans, the edible portions of those two crops should be assayed for heavy metals, particularly lead and cadmium.

4. Further area-wide assays for heavy metals of the fish in the Bayou are indicated to follow up on the mercury contamination found in the 1986 fish sample. If significant contamination is found, further action, such as posting warning signs about potential health hazards associated with the fish, must be considered.

5. A carefully planned off-site air monitoring program should be considered to detect possible major releases associated with emergency or other non-routine events at MSP. Such a program, if implemented, should include a local meteorology monitoring system to help provide additional insights on air emission migration patterns in the area.

6. In accordance with CERCLA, as amended, this site has been evaluated by the ATSDR Health Activities Recommendations Panel for appropriate public health actions. While it has not been possible to link adverse health outcomes to exposures from the MSP site, there is an indication that the population has experienced adverse health conditions. Therefore, the Panel has determined that follow-up public health activities are indicated for the community. Those activities will include a community health investigation and professional health education.

Public Health Actions:

Actions already undertaken - At the request of the Louisiana Department of Health and Hospitals, the Louisiana State University Medical School conducted a case-control study of the neuroblastoma cases that occurred in St. Mary Parish in 1986 - 1987.

Actions planned - The Public Health Action Plan resulting from this health assessment contains a description of actions to be taken by ATSDR and/or the Louisiana Department of Health and Hospitals in the community surrounding the site subsequent to the completion of this public health assessment. The purpose of this Public Health Action Plan is to ensure that this public health assessment not only identifies possible public health hazards, but also provides a plan of action designed to mitigate and prevent adverse human health effects resulting from exposure to hazardous substances in the environment. The public health actions to be implemented are as follows:

ATSDR will coordinate with the Louisiana Department of Health and Hospitals to undertake a community health investigation to better assess the community's concerns about adverse health outcomes that could be associated with exposures to hazardous substances in the environment. A Community Assistance Panel (CAP) will be established to ensure that the community has opportunity to contribute to the investigation. Specifically, the CAP will allow for information exchange during the planning and conduct of the investigation.

The Division of Health Education, ATSDR, will coordinate with the Louisiana Department of Health and Hospitals to conduct an education program that is proposed to advise public health professionals and the local medical community of the nature and possible consequences of exposure to contaminants in the environment.

ATSDR is currently working with the Louisiana Department of Health and Hospitals and the Louisiana Department of Environmental Quality on the Louisiana Database Development Project. That project would link health and environmental databases through a Geographical Information System (GIS). The project is not specific for the community around the Marine Shale Processors facility, but may assist in developing innovative ways to evaluate the relationships between environmental exposures and adverse health outcomes that could be applicable in the Morgan City/Amelia communities.

ATSDR is discussing implementation of recommendations 1 through 5 with the appropriate federal and state agencies.
