Granulomatous orchitis and epididymitis secondary to Histoplasma capsulatum and CMV in AIDS.

OBJECTIVES: To report a case of diffuse granulomatous orchitis and epididymitis secondary to histoplasmosis and CMV in AIDS. METHODS: A 35-year-old Argentinian homosexual male was evaluated for enlargement of the left testis, with induration of the left epididymis which did not respond to antibiotics. He had a history of intravenous drug use and antibodies to HIV-1, as well as a history of cerebral toxoplasmosis. A left facial ulcer and diffuse interstitial pulmonary infiltrate were also noted. The left cheek was biopsied and a left orchiectomy, and bronchoscopy were performed. RESULTS: Histologic examination of the enlarged testis and epididymis showed massive granulomatous orchitis and epididymitis with caseous necrosis. Yeasts diagnostic of H. capsulatum were seen and identified in culture. Viral inclusions consistent with cytomegalovirus were also noted. The skin biopsy showed granulomatous inflammation secondary to H. capsulatum. The bronchoscopy showed foreign body granulomatous inflammation, possibly due to talc. CONCLUSIONS: Patients with AIDS have an increased incidence of disseminated fungal infections including H. capsulatum. Although histoplasmosis may involve the testes and epididymis, massive involvement with destruction of these structures simulating a tumor is unusual, as is involvement with CMV.
