Taxonomy / License Information Page
Individual providers and organizations that render health care services will be required to indicate their taxonomy (provider/specialty type) on this page.
1. You must select a taxonomy by using the Add Taxonomy button to navigate to Select Taxonomy Page 1.

2. After you have made your taxonomy selection(s), a table on this page will display your taxonomy selection(s), along with a requirement to furnish a license number and State of licensure, if appropriate for that taxonomy(ies).

3. To remove a previously selected taxonomy, click the delete button for that taxonomy to remove it from your application.

4. Some organization providers may decide to obtain a single NPI, and not obtain NPIs for their subparts. If an organization provider takes this approach, that provider might want to furnish taxonomy codes that represent not only itself, but also its subparts if their taxonomies are different from that of the organization. Later on, if the organization provider determines that its subparts should have their own NPIs and the subparts obtain them, the organization provider should go into NPPES and update its record by deleting the taxonomies that describe its subparts.

5. Individuals who are sole proprietors may select taxonomies from either the Individual or the Non-Individual sections of the taxonomy code set. In the event that the answer to the sole proprietor question is changed from “Yes” to “No,” and the provider had selected a taxonomy code from both sections of the taxonomy code set, the NPPES will display error messages under each of the applicable taxonomies so that the provider will know which one(s) to delete to keep the NPPES record accurate.

6. If you select Multi-Specialty Group (193200000X) or Single-Specialty Group (193400000X) be aware that NPPES will require you to select the area of specialization (i.e. Internal Medicine, Family Practice) of the members of the group.

7. One of the taxonomies listed on this page must be selected as the Primary Taxonomy before continuing the application process.

8. If SSN or ITIN was reported as an Individual’s License Number for a Taxonomy that requires the reporting of a License Number, CMS has removed the License Number and has replaced it with the following text: "SSN/ITIN deleted" followed by a series of unique numbers.
9.If you enter a SSN/ITIN/EIN in the License Number field you will be shown a warning message that an invalid entry has been made and to update this field. If you chose to Save the invalid entry NPPES will convert it into a character string to protect the SSN/ITIN/EIN from being disseminated to downstream systems in clear text. If you entered a SSN it will be converted into 9 dollar signs ($), an EIN will be converted into 9 equal signs (=) and an ITIN will be converted into 9 asterisk (*). Only numbers that exactly match the SSN/ITIN/EIN entered on your record will be converted, if your SSN/ITIN/EIN report in NPPES changes the previously converted numbers will now display.

NOTE: A provider may not select more than 15 taxonomies when applying for an NPI. DO NOT report the Social Security Number (SSN), IRS Individual Taxpayer Identification Number (ITIN) in the License Number field.
