The Parent Case Management Program is funded through a contract with the Texas Dept. of Health to provide independent, comprehensive case management for children with special health care needs. They serve children from birth through age 21 in a 32 county area surrounding Abilene and San Angeo, TX. All staff have children with special needs themselves. They assist families with information, applications for programs, appointments with providers, arranging transportation, needs and lodging, obtaining equipment, medication and other needs. They assist families with their child's medical, educational or social services needs. There is no charge.
